Areas Inspected:

Routine announced safety inspection of the licensee's inserviceinspection(ISI)effortincludingprogramreview(73051), procedure review (73052), data review (73755), and observation of work activities (73753).

Results: No violations or deviations were' identified. Based on the results of-the inspection, the NRC inspector noted the following:

The implementation of the ISI program followed the requirements of ASME Section XI and the licensee's program.

All activities were controlled by well stated and defined procedures.

The staff's of both the licensee and ISI contractor were knowledgeable and competent.

In general, the' inspection results indicate a continuing good

performance.

Inservice Inspection (ISI)

a.

Review of Program (73051)

LMT, Conam, and Zetec performed the ISI in accordance with ASME Section XI, 1980 Edition, Winter 1981 Addenda. There was sufficient organizational staff to ensure that acceptable ISI work was performed.

Where ASME requirements were determined to be impractical, specific relief requests were requested from the office of Nuclear Reactor Regulation (NRR). The NRC inspector reviewed the specific relief requests including the related correspondence between the licensee and the NRC and had r.o questions.

The inspection scope for this outage consisted of 88 ultrasonic examinations (UT), 68 liquid penetrant (PT) examinations, 32 magnetic particle (MT) examinations, 134 VT-1 visual and 110 VT-3 visual (VT) examinations.

The NRC inspector found the ISI program to be properly implemented.

Evaluations of the above examination data were found to be conservative.

As a result of the steam generator tube examinations, the following

actions were taken:

Three tubes were plugged in steam generator No. 11 due to

support plate tube thinning on the cold leg side. With this L

addition, a total of 2.13% of the tubes are plugged.

The examination of steam generator No. 12 resulted in the

detection of 49 new indications.

Forty-one indications were evaluated as tube sheet stress corrosion cracking, seven were evaluated as thinning on the cold leg side, and one on the hot leg,ide.

Of these, 37 tubes were sleeved and twelve were plugged resulting in a total of 1.87% of the tubes in.this steam generator being plugged.

Additional work on the steam generators included the removal of a total of 81 plugs considered susceptible to cracking.

Some tubes were returned to service by sleeving while others were replugged.

No violations or deviations were identified.

3.

The inspector met with site representatives (denoted in Paragraph 1) at u

the conclusion of the inspection. The inspector summarized the scope and findings of the inspection noted in this report.
