An NRC inspection was conducted May 26 through July 6, 1996, at your Diablo Canyon Nuclear Power Plant, Units

and 2 reactor facilities.

The enclosed report presents the scope and results of that inspection.

During the inspection period, your conduct of activities at the Diablo Canyon facility was generally characterized by safety conscious operations and good maintenance and engineering practices; however, two violations were identified.

The first violation, although identified by your staff, is being cited since it involved a failure to follow the precautions in a surveillance test procedure which resulted in two residual heat removal pumps being technically inoperable for almost an hour.

Several individuals were involved with both the approval and performance of the surveillance test; however, none of them identified the incompatibility of performing the surveillance test with ongoing maintenance activities.

This indicated the lack of a questioning attitude.

The second violation involves the failure to remove Action Request stickers from the control room panels when deficiencies were corrected, as required by procedures.

The inspectors noted five examples.

Your staff had previously identified a number of examples but had not been determined the situation to represent a programmatic problem.

The failure to remove the stickers created the potential for the operators to take actions based on inaccurate information regarding equipment availability during both normal and abnormal operations.

The previous actions taken in response to this problem by your staff appear to have been ineffective since similar problems continue to occur, and also several additional instances were identified by your staff after the end of the inspection period.

These violations are cited in the enclosed Notice of Violation (Notice)

and the circumstances surrounding the violations are described in detail in the enclosed reports Please note that you are required to respond to this letter
