NOTICE OF VIOLATION (INSPECTION REPORT NOS.- 50-269/92-14, 50-270/92-14 AND 50-287/92-14)
This refers to the inspection conducted by Mr. P. E. Harmon during the period May 24 - June 19, 199 The inspection included a review of activities authorized for your Oconee facility. At the conclusion of the inspection, the findings were discussed with those members of your staff identified in the repor The report documenting this inspection was sent to you by letter dated June 29, 199 NRC concerns relative to the inspection findings were also discussed in an enforcement conference held on July 17, 1992, in the Region II office, Atlanta, GA. A list of attendees and a copy of your presentation materials are enclose Based on our evaluation of this inspection and the information provided at the enforcement conference, certain activities appeared to be in violation of NRC requirements, as specified in the enclosed Notice of Violation (Notice). The violations have been categorized at Severity Level IV because of the low safety significance of this event, and the fact that operators retained the ability to manually operate the affected Emergency Feedwater train from the control room, and because of the availability of other sources of feedwater. These violations are nevertheless of concern because they indicate-weaknesses in your post trip review and event followup processes, especially since these problems were identified by the NRC resident inspector and not your staf Because an inadequate post trip review was performed, the "A" train of the Emergency Feedwater System was inoperable and went undetected during plant startup and operatio You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response.
