INSPECTION SUMMARY: Inspection conducted February 1 - 4,1993 (Inspection Report 50-245/93-04)
AREAS INSPECTED: A regional initiative safety inspection was conducted of the licensee's licensed operator training program by observing classroom training and reviewing training materials.
RESULTS: Based on the observed classroom training and the review of training materials, training of licensed operators is adequate to assure safe pir.nt operations.

displayed positive attitudes. The inspector determined that the one emergency operating procedure (EOP) revision was needed, and licensee personnel properly responded by making the revision during the inspection. The error was apparently an administrative oversight.
One apparent violation was identified related to the failure to conduct independent and comprehensive Quality Assurance audits of the licensed operator training programs (paragraph 4.0, 245/93-04-01). This resulted in missed opportunities to detect and correct the known programmatic training problems.
The licensee's program for maintenance of active and inactive licensed operators satisfied the -
regulatory requirements and was comprehensive. The licensed operator requalification (LOR) training program implementation procedure is currently in transition. The operator training staff provides the necessary resources to support safe operation of the plan,

1.0 INTRODUCTION AND OVERVIEW -
i In September 1991 and again in September 1992, the NRC found the licensed uperator b
requalification training program at Millstone 1 to be unsatisfactory. As a result, Region I j

- initiated additional inspections to assure continuing high quality training oflicensed operators
.j with respect to safe plant operations.
2.0 OBSERVATIONS OF CLASSROOM TRAINING

Scope The scope of the inspection was to determine the adequacy of training conducted of licensed operators in a classroom setting. The inspector observed the following licensed operator classroom topics; emergency operating procedures (EOPs), electrical review, enforcement

training, and plant shutdown risk. In addition, the inspection was to determine the adequacy l
of training materials used by the facility instructors to conduct the training.

inspector determined the training was conducted in a professional manner by the facility,

instructor and the plant operating crew was receptive to the training given. The inspector -
noted the computer aided display of questions, student responses, and the correct answers aided the training process by stimulating discussions.'

The inspector reviewed the materials used by the facility instructor to conduct the observed

training. The inspector determined the training materials were adequate. However, the-inspector determined that EOP-575, " Failure To Scram," Rev 7, was missing a hold point at step FLD-5. The facility took prompt corrective actions and revised the EOP to include the

appropriate hold point. Also, the facility reviewed all other EOPs and verified similar problems did not exist.

The classroom instructors were well prepared and very knowledgeable about the training -
l topics. The instructors stimulated discussions when the operators questioned specific training
information. The computer driven questions and answers promoted total class participation -
l and kept the operators focused on the important knowledge objectives. The operators.
i displayed a positive attitude toward the new training techniques.

The inspector observed the shutdown risk classroom training for licensed and non-licensedi
. operators. The instructor emphasized the potential problems associated with shutdown
operation. The training included recent shutdown procedure changes to provide the written i

plant management philosophy to reduce the probability of shutdown problems. The presentation included information about actual plant shutdown problems at Millstone and other facilities to characterize the safety significance of shutdown events.

Conclusions Classroom instructors were professional and the classroom training was adequate. In addition, the training materials were adequate and the students displayed positive attitudes.

3.0 LICENSED OPERATOR TRAINING RELATED ITEMS Scope The inspector reviewed the Nuclear Training Manuals associated with the licensed operator

requalification training (LORT) implementing procedures, records and staffing. The review
was focused on the facility's ability to administer and document the training programs in accordance with regulatory requirements. The review included the operations department program for maintenance of active and inactive licensed operators.
Findings The description of Millstone I licensed operator requalification (LOR) training programs are contained in NTM-3.028, " Millstone I Licensed Operator Requalification Implementing
'
Procedure," Rev. 3. The LOR training program procedure is currently in transition due to the recent changes in the LOR program.
The inspector reviewed the licensee's control of active and inactive senior reactor operator (SRO) and reactor operator (RO) licenses. An active licensed operator must participate in the requalification training program and be in compliance with all conditions of the license as stated in 10 CFR 55.53.(e) including watchstanding. The NRC regulation states that an-active SRO shall perform the functions of a senior operator by standing a minimum of seven 8-hour or five 12-hour shifts per calendar quarter.

The operation department instruction 1-OPS-8.05, " Maintenance Of Operator Licenses,"
Rev. 3, provided a comprehensive standard for the maintenance and restoration of active NRC licenses. The procedure clearly describes the necessary requirements that a licensed operator must meet to comply with the regulations.
The inspector reviewed the current Millstone I operator training depanment staff complement. The training depanment currently supplements their own staff with qualified contract personnel. The contract personnel are administering classroom training and

implementing program changes. The department has hired additional personnel with operator training experience to replace the contract personnel. The newly hired personnel are undergoing unit specific training and qualification.
Conclusions The licensee's program for maintenance of active and inactive licensed operators satisfied the regulatory requirements and was comprehensive. The LOR training program implementation procedure is currently in transition. The operator training Pff provides the necessary resources to support safe operation of the plant.
4.0 LICENSEE ACTIONS ON PREVIOUS FINDINGS (CLOSED) Unresolved item (UNR 50-245/92-32-02) Audits of the Licensed Operator Trainine Programa The inspector reviewed the quality assurar. ? and other licensee performed independent audits of operator training programs with the licensee. The inspector also reviewed how the technical specification audit requirements of paragraph 6.5.3.7 were being met. Technical Specification 6.5.3.7.b. requires that the Nuclear Review Board perform audits that encompass: the performance, training, and qualifications of the unit staff at least once per 12 months.
The Northeast Utility Qudty Assurance Program further defines the a':diting of the unit staff by commitment to ANSI N18.7/ANS-3.2-1976. Section 4.5 of this ANSI, " Audit Program,"
states, in part, that audits of selected aspects of operational phase activities shall be performed with a frcquency commensurate with their safety significance and in such a manner as to assure that an audit of all safety-related functions is completed within a two year period. Audits shall include as a minimum verification of compliance and effectiveness

of implementation of internal rules, procedures, regulations and license provisions; programs
'
for training, retraining, qualification and performance of the operating staff [ comprehensive review). In addition, Section 2.0, " Definitions," defines the onsite operating organization'as,
"Onsite personnel concerned with operations, maintenance and certain technical services."
The inspector requested a copy of the operation training audits from 1987 to 1993. The quality assurance personnel produced two documents. First, was an audit of the emergency operating procedures (EOPs) QSD-90-4326, dated June 14, 1990.' The audit only verified that EOPs have been incorporated into the operator training program. The audit did not meet the ANSI N18.7 criteria noted above for a planned and comprehensive training audit.. The second audit, was a Surveillance of Millstone I licensed operator requalification training records No. A-324, dated March 9,1992, at the request of the Operator Training Manager.

The inspector concluded that in the period of January 1,'1987, to January 1,1993, the licensee did not perform any planned, independent and comprehensive audit of the operator

training programs every two years as collectively required by both the TS and the ANSI
,
standard. This is an apparent violation of technical specification 6.5.3.7.(b), and ANSI N18.7,1976, sections 2 and 4.5 (245/934)4-01).
The following two items are examples of problems that could have possibly been detected and addressed by an independent and comprehensive audit. First, the weaknesses identified in the 1991 and 1992 unsatisfactory NRC staff determination might have been identified in a more timely manner before NRC identification. Second, the NRC identified documentation problems with Millstone I training records (inspection conducted in January 1992) might also have been identified in a more timely manner. The licensee's failure to perform the audits.
resulted in several missed opportunities over at least a five to six year period to identify and correct significant deficiencies in the licensed operator training program.
The unresolved item (URI 245/92-32-02) for performance of the operator training audits required by ANSI N18.7 and Technical Specification 6.5.3.7(b)is considered closed and replaced by the appaient violation (245/93-04-01) documented herein.

5.0 UNRESOLVED ITEMS Unresolved items are matters about which more information is required in order to determine -
whether they are acceptable, a violation or a deviation. Unresolved items disclosed during the inspection are discussed in Section 4.0.
