SUBJECT: NRC INSPECTION REPORT NO. 50-245/9M)4
'
This letter transmits the findings of the special safety inspection conducted by
Messrs. S. L. Hansell and C. E. Sisco of this office from February 1 - 4,- 1993, at the
Millstone Unit 1 Nuclear Power Station, Waterford, CT. At the conclusion of the -
inspection, the findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within these areas, the -
.
inspection consisted of observations of licensed operator training, selective review of
procedures, and interviews with personnel. The operator training programs are directly
related to the safe operation of the plant.
As a result of this inspection, we found that training administered to licensed operators is
conducive to safe plant operations.
Based on the results of the inspection, certain of your activities appeared to be in violation of
NRC requirements, as specified in the enclosed Notice of Violation (Notice). The violation
is of concern because of the failure to perform audits of the Unit 1 operating staff which
could have alerted plant management to training program deficiencies before identification by

NRC staff during recent licensed operator requalification program evaluations.
You are required to respond to this letter and should follow the instructions specified in the
_
enclosed Notice when preparing your response. In your response,' you should document the
specific actions taken and any additional actions you plan to prevent recurrence. After
reviewing your response to this Notice, including your proposed corrective actions and the
results of future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
