SUMMARY: Requalification written examinations and operating tests were administered to twelve senior reactor operators (SROs) and one reactor operator (RO). These examinations were administered in accordance with Revision 7 to NUREG-1021. Twelve of the operators passed their examinations. One of the SROs failed the written portion of the examination and was removed from licensed duties. Seven licensed operators who had previously taken

and passed the NRC requalification examination were added to the thirteen operators identified above in order to make up four cre.ws consisting of five operators per crew for the simulator examinations. All crews operated successfully during the simulator scenarios. The results from this examination were used to perform a program evaluation of the requalification training program.

Based upon the results of 20 licensed operators having been examined and 19 successfully passing all portions of the examination and including the evaluation of 4 operating crews during the simulator evaluation, Millstone 2's licensed operator requalification training program was determined to be satisfactory.

One of the SROs tested failed the written portion of the requalification examination.
Northeast Utilities removed the individual from licensed duty and will take corrective action consistent with the provisions of their requalification program before returning him to licensed duty. The SRO still must pass an NRC-administered requalification examination to be eligible for license renewal.
2.0 PROGRAM EVALUATION RESULTS 2.1 Background From May 24 - 27,1993, the NRC reviewed the proposed examination with MP-2 training and operations representatives at the MP-2 site. This detailed review included a review of the adequacy of the written examination questions, a walkdown of all job performance measures (JPMs) and the validation of the simulator scenarios. The validation of the simulator scenarios included a review of expected operator actions and the validity of designated crew critical tasks. Minor changes were made to the proposed exam; for example, some of the distractors in the written exam questions 'were changed. The excess steam demand simulator scenario was modified slightly to provide a better opportunity for the operators to take action to prevent the possibility of pressurized thermal shock.

2.2 Overall Rating: Satisfactory The Millstone Plant, Unit 2 (MP-2) program for licensed operator requalification training was rated satisfactory in accordance with the criteria established in Revision 7 of ES-601,
" Administration of NRC Requalification Program Evaluation." Those criteria are as follows:

At least 75% of all operators pass all portions of the examination. This number

includes operators who participate in the simulator examination for the purpose of meeting crew composition requirements.
NRC grading is the only consideration for this criterion. There were seven individuals who participated only in the simulator portion of the examination in order to meet crew requirements. Nineteen of twenty operators (95%) passed the examination.
B.
At least two-thirds (66%) of the crews pass the simulator examination.
NRC grading is the only consideration for this criterion. Four crews were evaluated, and all four crews (100%) passed the simulator portion of the operating examination.
The requalification program met these criteria and was rated as satisfactory.
2.3 Programmatic Strengths and Weaknesses A.
Strengths:
1.
MP-2 training staff was very knowledgeable in all aspects of the exam process.
Due to NRC commitments the scheduling of the examination preparation week was changed to the week immediately prior to the examination. The training staff was very understanding of the schedule change and cooperated fully with the exam team. All changes made during the preparation week were incorporated for the examination the following week.

The crews examined on the simulator demonstrated good teamwork and provided each other with good information, which assisted in correctly responding to the simulator conditions.
B.
Weaknesses:

1.
Some of the JPMs that were used for the examination were not written as detailed as the procedure for the task. An example of this was the JPM that tested the ability to plac e the "C" battery charger in service. The lack of detail in the JPM may lesd to confusion for the evaluators when the operator is performing tasks not outhned in the JPM.
2.
The question about shutdown margin requirements on the static exam was missed by five people. MP-2 static exam item number 03691 asks, "What minimum RCS boron concentration must exist (at 205 F) before boration may

3.0 EXIT MEETING On June 4,1993, at the conclusion of the requalification examination, the NRC conducted an exit meeting at the Millstone training facility. Those personnel in attendance are noted in paragraph 4.0 of this report. Examination development and conduct, general observations noted during the exam, and programmatic strengths and weaknesses were discussed.
Examination results, as determined by both the NRC and MP-2, were also discussed.

Dear Mr. Bettenhausen:
During the week of May 31, 1993, Licensed Operator Requalification Examinations were administered to twenty Millstone Unit 2 Licensed Operators and Senior Licensed Operators.
Of this number, thirteen licensees were administered full scope examinations for the purpose
of license renewal. Seven additional licensees participated in the dynamic simulator examinations for the purpose of completing crew complements.
These examinations were conducted in accordance with the applicable sections of Revision 7 of NUREG 1021, Operator Licensing Examiner Standards.
Accordingly, the examinations were prepared, administered, and evaluated by both NRC and facility examiners.
One examinee failed the written examination.
He was removed from licensed duties immediately. He will be remediated and._ required to pass a written re-examination prior to being returned'to l'icensed duties.
Preliminary results of the facility evaluations for all portions of

the examination were provided to Mr.
Kerry Ihnen, NRC Chief Examiner, on June 4,
1993.
Based on our review of the exam grading, these results can be considered final.
Attached is a summary of our grades.

necessary remediation and enhancements to the MP2 Licensed Operator Requalification Program content.

o Teamwork and crew interaction.
Where appropriate, team members were involved in decision-making and shared, with each other, information concerning event strategy and inter-watchstation operations.
Individual team members operated within their pre-defined roles.

o Crew concern for plant personnel safety.

WRITTEN EXAMINATION Examinee performance on the written examination was generally very good to excellent for nine individuals.
Three examinees scored in the low 80's and there was one failure.
Four questions, (3 in Section A and 1 in Section B), were missed by a significant number of examinees, (>25%).
Analysis of the questions and incorrect answers chosen indicates that examinees may not have read the questions carefully or considered all of the ramifications imposed by conditions rather than a generic weakness.

ATTACIIMENT 2 SIMULATION FACILITY REPORT Facility Licensee: M P-2 Facility Docket No: 50-336 Requalification Examinations Administered from: June 1 - 3,1993 This form is used only to report observations. These observations do not constitute audit or inspection findings and are not, without further verification and review, indicative of non-compliance with 10 CFR 55.45(b). These observations do not affect NRC certification or approval of the simulation facility other than to provide information that may be used in future evaluations. No licensee action is required in response to these observations.
During the conduct of the walk-through portion of the operating tests, the following item was observed.
ITEM DESCRIITION Faulted aux feed JPM During the walkthrough portion of the examination, a JPM was used that involved a malfunction such that the surveillance test performed on the auxiliary feedwater pump.would fail. The malfunction was correctly entered into the simulator computer; but, when the JPM was performed, the test did not fail. When tested during the prep week, this did not occur.
