SUMMARY: Examination / Inspection conducted February 11 and 12.1993 Onspection Report 50-245/93-06(OL))
AREAS INSPECTED: Routine announced examination / inspection to administer a retake
requalification examination to one senior operator. The inspection reviewed licensee actions
in planning and completing individual remediation plans for operators who have not yet passed an NRC administered requalification examination.

RESULTS: The individual passed the retake requalification examination by properly I
classifying a simulator scenario in accordance with station emergency classification guides.

Individual training plans were found to be in place and being implemented in remediating
operations staff who have not yet passed the NRC requalification exam.

1.0 INTRODUCTION Examination report 50-245/92-23 (OL) documented the unsatisfactory Millstone Unit I
'
requalification program evaluation. Following the evaluation, a Confirmatory Action 12tter (CAL) 1-92-014 was issued that restricted those licensed operators who failed either the NRC
or facility requalification examination until either the facility requalification program was
determined to be at least provisionally satisfactory or until the licensed operators have been
evaluated as successful by the NRC. On February 12, 1993, the NRC administered a
,
simulator scenario wherein an SRO was required to demonstrate the ability to correctly l
classify an event. The examiner used the process and criteria described in NUREG-1021,
" Operator Licensing Examiner Standards," Revision 6.
2.0 INDIVIDUAL EXAMINATION RESULTS I
i The scenario involved a small loss of coolant accident coincident with a partial loss of i
electrical power to vital plant components. The individual correctly classified the event as an f
Alert-Charlie 1, in accordance with station emergency plan classification guides.
3.0 INSPECTION FINDINGS.

As a corrective action for the 1992 individual requalification examination failures, the
)
Millstone Unit I training department has developed and implemented individualized trainmg r
plans for each of the operators. The inspector reviewed the training plans for five operators
'
yet to successfully complete NRC administered requalification examinations. Completion of the actions specified in the individual plans was verified by discussions with training staff and i
review of records. The inspector also attended a staff training session dealing with
implementation of the Information Referenced Testing method now being used at Millstone i
'
Unit 1. The training session focused on development of the examination method, implementation, difficulties, and expected feedback to trainers on training effectiveness. No

weaknesses were identified by the inspector. A more thorough assessment of training at the
unit will be completed in a future inspection.
6.0 EXIT MEETING

At the completion of the inspection and examination on February 12, 1993, the inspector

discussed results with Mr. R. Heidecker, Mr. M. Ross, and Mr. P. Prezkop, all of Millstone
Unit 1.
