On March 31, 2020, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at R.E. Ginna Nuclear Power Plant, LLC. On April 15, 2020, the NRC inspectors discussed the results of this inspection with Mr. Paul Swift, Site Vice President and other members of your staff. The results of this inspection are documented in the enclosed report.

The NRC inspectors did not identify any findings or violation of more than minor significance during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at R.E. Ginna Nuclear Power Plant, LLC, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Ginna began the inspection period at rated thermal power. On March 26, 2020, the unit began gradually lowering power as it entered end-of-cycle coast down operations for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/readingrm/doc-collections/insp-manual/inspection-procedure/index.html.

Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. From January 1 - March 19, 2020, the inspectors performed plant status activities described in IMC 2515, Appendix D, Plant Status, and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Starting on March 20, 2020, in response to the National Emergency declared by the President of the United States on the public health risks of the coronavirus (COVID-19), resident inspectors were directed to begin telework and to remotely access licensee information using available technology. During this time the resident inspectors performed periodic site visits each week during which they conducted plant status activities as described in IMC 2515, Appendix D, and observed risk significant activities when warranted. In addition, resident and regional baseline inspections were evaluated to determine if all or portions of the objectives and requirements stated in the IP could be performed remotely. If the inspections could be performed remotely, they were conducted per the applicable IP. In the cases where it was determined the objectives and requirements could not be performed remotely, management elected to reschedule the inspection to a later date.

71111.01 - Adverse Weather Protection

(1) The inspectors observed and evaluated licensed operator performance in the main control room during an announced fire drill on January 29, 2020 and operators' procedure use and adherence to perform a reactor coolant system alternate dilution, nitrogen make-up to 'B' safety injection accumulator and shift turnover on February 24, 2020.

Licensed Operator Requalification Training/Examinations (IP Section 03.02) (1 Sample)

(1) The inspectors observed and evaluated licensed operator performance in the simulator during licensed operator requalification training on March 2, 2020. The training involved a scenario that contained, but was not limited to, a dropped control rod, fuel failure, loss of all offsite power, faulted steam generator, and a failure of containment recirculation fan coolers coincident with a failure of containment spray automatic initiation signal.

60854.1 - Preoperational Testing of Independent Spent Fuel Storage Facility Installation at Operating Plants Preoperational Testing of Independent Spent Fuel Storage Facility Installation at Operating Plants

(1) The inspectors evaluated Exelons performance during NRC observed preoperational dry run activities that were performed in order to fulfill requirements in the Certificate of Compliance (CoC) No. 1014, Condition 10. The inspectors observed dry run activities on March 16-17, 2020. Specifically, the inspectors observed or reviewed the following activities;
  • Closure welding of an MPC mockup including lid to shell, port covers, and closure ring
  • Non-destructive weld evaluations including visual and penetrant testing
  • Simulated radiological field surveys and RP coverage of welding activities
  • Reviewed a recording of PCI welding personnel cutting open a welded canister mock-up removing the port covers and lid that was witnessed by NRC personnel

INSPECTION RESULTS

Observation: PI&R Sample Inspection Regarding Pump Bearing Oil Sampling 71152 Process The inspectors reviewed Exelons evaluation, extent of condition reviews, and corrective actions associated with two problems involving safety related pump oil sampling and analysis. The first problem was documented in AR 04278159 and involved a safety injection pump lubricating oil sample that was not submitted for analysis as required by Ginna procedures. The second problem was documented in AR 04259000 and involved a low viscosity result from a turbine driven auxiliary feedwater pump inboard bearing lubrication oil sample. Exelon determined the oil sample for the 'A' safety injection pump was not submitted for sampling due to a human performance error and was an isolated event. Additionally, Exelon determined the low viscosity reading on the turbine driven auxiliary feedwater pump was likely a sampling error in that the oil sample was likely taken from the turbine bearing instead of the pump bearing. The readings obtained were consistent with normal readings for the turbine lubricating oil. Exelon's corrective actions included revisions to the labeling process for lubricating oil samples and training to applicable personnel. Exelon determined operability of the safety injection pump and the turbine driven auxiliary feedwater pump was not impacted since the pumps oil reservoirs are small such that a full oil replacement is done every time an oil sample is taken. The inspectors performed visual inspections of all the safety injection and turbine driven auxiliary feedwater pumps, interviewed Exelon's system engineer, the component maintenance optimization engineer, and reviewed applicable safety injection and turbine driven auxiliary feedwater pump vibration and lubricating oil data. In addition, the inspectors verified the safety injection and turbine driven auxiliary feedwater pump surveillance test results were satisfactory and vibration data and bearing temperatures remained normal to date. The inspectors concluded the problem evaluations identified the likely causes and the corrective actions addressed the causes. Additionally, the inspectors affirmed the extent of condition by independently reviewing associated corrective actions issues involving Exelons lubricating oil process at Ginna for the last three years. The inspectors did not identify a negative trend in human performance issues.
