On June 30, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Dresden Nuclear Power Station, Units 2 and 3. On July 26, 2023, the NRC inspectors discussed the results of this inspection with P. Boyle, Site Vice President, and other members of your staff.

The results of this inspection are documented in the enclosed report.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Dresden Nuclear Power Station, Units 2 and 3, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors.

PLANT STATUS

Unit 2 began the inspection period at rated thermal power. On April 25, 2023, the unit was shut down to perform replacement of the 2A recirculation pump seal. Following replacement and retest, the unit was synchronized to the grid on April 28, 2023, with full power achieved that same day. On May 12, 2023, the licensee discovered the 2/3 F lift pump in a tripped condition.

The licensee reduced power to approximately 94 percent in order to troubleshoot and perform maintenance on the 2/3 F lift pump. The unit was returned to full power operations on May 13, 2023. On May 21, 2023, power was reduced to approximately 83 percent to perform routine rod sequence exchange and surveillance testing. The unit was returned to full power the same day and remained at or near full power for the remainder of the inspection period.

Unit 3 began the inspection period at rated thermal power. On April 21, 2023, the unit was shut down to perform replacement of the 3A recirculation pump seal. Following replacement and retest, the unit was synchronized to the grid on April 24, 2023, with full power achieved that same day. On May 12, 2023, the licensee discovered the 2/3 F lift pump in a tripped condition.

The licensee reduced power to approximately 91 percent in order to troubleshoot and perform maintenance on the 2/3 F lift pump. The unit was returned to full power operations on May 13, 2023. On May 26, 2023, power was reduced to approximately 17 percent in order to facilitate repairs to channel one of the automatic voltage regulator system. During repair activities the licensee caused a loss of channel two, a complete loss of the automatic voltage regulator system and subsequent generator trip. The reactor remained online at approximately 17 percent power. The licensee replaced both channels of automatic voltage regulator and synchronized the generator to the grid on May 17, 2023. The unit was returned to full power the same day and remained at or near full power for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed onsite portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

(1) The inspectors reviewed the licensees corrective action program for potential adverse trends in issue identification and documentation that might be indicative of a more significant safety issue.

INSPECTION RESULTS

Observation: Adverse Trend in Corrective Action Program Issue Identification and Documentation 71152S The inspectors performed a semiannual review of issues entered into the corrective action program and a cognitive review of plant observations over the period of January 1, 2023, to June 30, 2023, to identify any potential trends that might indicate the existence of a more significant safety issue. In the first half of 2023, the NRC inspectors identified several examples of issues not being identified at a low threshold and documented within the corrective action process.

The first example occurred on February 1, 2023, during walkdown of the Unit 3 station blackout (SBO) diesel generator when the inspectors identified that the air start motor cover had been left off and resting on the ground. The inspectors questioned what activity the cover had been left off as the guard is left in place to provide a barrier to individuals from rotation machinery during operation and represented a potential safety concern. The licensee documented the inspectors questions in CR 4551943, NRC Concern U3 SBO Area.

Following review of licensee documentation, the inspectors determined that the cover had been left off following post-maintenance run of the SBO diesel generator on January 13, 2023. The inspectors noted that the licensee inspects the area at least twice a day during operator rounds and had a multitude of opportunities to identify the discrepant condition during the 19 days the cover had been left off.

The second example occurred on March 9, 2023, during routine tour of the Unit 2 reactor building. The inspectors noted indications of an active leak on the 2B3999600 RBCCW heat exchanger tube side drain valve. The inspectors noted that directly below the leak location a discolored indication on the floor appeared to be associated with the leak. The inspectors raised the issue with the operations staff. Following review of the licensees documentation, the inspectors concluded that the leak had not been previously identified and likely had been occurring for some time due to the indications noted on the floor and relative slow leak rate of one drop per minute. The inspectors determined that the leak rate did not represent a challenge to the functionality of the heat exchanger or the Unit 2 RBCCW system. The inspectors noted that the licensee implemented procedure MAAA716010-1103, Fluid Sealing Technology Program, and hung a leak monitoring tag near the leak location.

The inspectors concluded that due to leak rate, floor discoloration and required twice-aday plant walkdowns, the licensee had multiple opportunities to identify and document the discrepant condition before being discovered by the inspectors.

The third example occurred on May 18, 2023, during routine walkdown of the Unit 3 reactor building. The inspectors observed several valves associated with the west bank of hydraulic control units (HCU) had various states of leak indications, annotations, and flagging. The inspectors noted several valves in question that had ongoing leak indications and questioned the licensee staff to understand corrective action status for those leaks. During review, the inspectors determined that two valves had not been identified and documented with the corrective action process, and the others were in various stages of planning within the work planning process. The inspectors determined that the licensees work planning process had created a universal work order to address HCU leaks for which some remained in planning status for up to five refueling cycles. The inspectors questioned why this was and was provided evaluation EC 395859, Evaluation of Leakage at Mechanical Connections for Class 1(2) System Leakage Test, revision 2, which established the threshold of 60 drops per minute as acceptable and would not challenge component function. The inspectors determined that while they did not identify an issue with the licensees developed threshold, based on their field observations, implementation of procedure MAAA716010-1103 seemed inconsistent as the purpose was to establish a process for Fluid Management that ensures external leakage from plant systems is classified, prioritized, monitored, managed, and corrected in a timely manner. The inspectors concluded that unrecognized or unmonitored leaks could challenge equipment reliability. To that end, the inspectors noted that on June 16, 2023, the licensee documented CR 4685191, PS 3-305-42-31 Not Working Properly Due to Water Intrusion, and CR 4685217, PS 3-305-46-27 Not Working Properly Due to Water Intrusion, which identified that during performance of DIS 030011, Control Rod Drive Accumulator High Level Switch Functional Test, for their respective HCUs that isolation valves with noted leaks above the switches had leaked in and affected the function.

The fourth example occurred on May 17, 2023. During routine plant walkdowns, the inspectors identified a section of hose stored underneath the staircase in Unit 2 west corner room with no associated automatic sprinkler system associated. The inspectors noted the materials to the operations staff and determined that procedure OPAA201009, Control of Transient Combustible Material, Revision 28, Section 4.4.1.5, stated in part, do not stage or store transient combustible material that is inside or beneath stairways, unless the area underneath the stairway is protected by an operable automatic sprinkler system. The licensee promptly removed the material and documented the inspectors concerns in CR 4678881, NRC ID: Found a Hose Underneath Staircase U2 West Corner Room. On June 13, 2023, during routine turbine building tours, inspectors identified transient combustible materials staged to perform Unit 3 seal oil vacuum pump maintenance were stored underneath a stairwell with no associated automatic sprinkler system. The licensee promptly removed the material and documented the inspectors concerns in CR 4684532, NRC ID: Transient Combustible. Additional actions taken by the licensee were to formally communicate transient combustible control requirements through site wide communications contextualized to risk importance and safety implications. In both examples, inspectors determined that the licensees failure to implement Section 4.4.1.5 of procedure OPAA201009, revision 28, were performance deficiencies. Specifically, combustible loading material was staged underneath stairways without associated automatic sprinklers.

The inspectors screened the issues using IMC 0612 Appendix E, Examples of Minor Issues, and determined that the performance deficiencies were similar to example 4.j in that the licensee failed to complete an engineering evaluation to compensate for all transient combustibles in an area which was contrary to Dresden Nuclear Power Station, Unit 2 Amendment to Renewed Facility Operating License Condition 2.E and Dresden Nuclear Power Station, Unit 3 Renewed Facility Operating License No. DPR25 Condition 3.G, and failed to implement all provisions of the approved fire protection program as described in the Updated Final Safety Analysis Report. The inspectors reviewed the circumstances, materials and potential impacts and determined that they did not adversely affect the mitigating systems cornerstone objective because the failures to implement OPAA201009 had little or no safety impact. Specifically, the transient combustibles could not affect equipment important to safety and did not exceed any licensing basis requirements. The inspectors determined that the transient combustibles identified were well below the fire hazards analysis limits. Upon identification, the licensee took immediate action and restored compliance. The failures to comply with Dresden Nuclear Power Station, Unit 2 Amendment to Renewed Facility Operating License Condition 2.E and Dresden Nuclear Power Station, Unit 3 Renewed Facility Operating License No. DPR25 Condition 3.G constitutes a minor violation that is not subject to enforcement action in accordance with the NRCs Enforcement Policy.

Finally, in conversation with plant management, the inspectors discussed the previously noted issues and ongoing efforts by the licensee to address equipment reliability. The inspectors noted that challenges in discrepant condition identification could also present challenges to, and hamper efforts to broadly address and improve equipment reliability. Other than the previously dispositioned issue, the inspectors did not identify any findings or violations associated with the inspection activities.
