improper control of-equipment status through your Blue Markup control process,

This violation has been

categorized as Severity Level IV _ according to 10 CFR Part - 2, Appendix C ~

(Enforcement Policy).. This violation. concerns failures of.your staf f to main-tain' positive contral of system status.. It also id:ntifies an incomplete root
< cause analysis following the May 21,1990: feedwater pump suction.- valve event, in that the-correction actions did not identify additional Blue Markup < con-trolled components which were out of position and were subsequently discovered by the inspectors'.
We 'are concerned that the root cause analysis focused on
.
personnel performance and did not evaluate the effectiveness : of the tagging process.
