No package was opened.

A l representative of the State of Washington was onsite.

Three l inspector hours were expended on this effort.

i ! Results: An apparent item of noncompliance with NRC/ DOT regulations was identified.

See paragraph 7.

The appropriate organizations to take enforcement actions appear to be the NRC Region IV and the Department'of Transportation.

Licensee - Shipper: A) U.S. Geological Survey Denver Federal Center Denver, Colorado , 3) U.S. Department of Agriculture SEA-AR Denver, Colorado - Shipment Inspected: June 18, 1980 Carrier: Garrett Freight Lines Trailer: 42-4672 , Freight Bill: GBL-L8882904 , Garrett Freight Lines, freight fowarder, delivered seventeen metal , } containers to NECO. All containers had a breakable wire seal.

Six containers were from.the U.S. Department of Agriculture, Denver, Colorado.

These 55 gallon drums contained a total of 14 millicuries of H-3, I-125 and C-14 The containers were properly labeled as Radioactive White I.

The U.S. Geological Survey, Denver, Colorado, forwarded eleven 55 gallon containers. A total of about 27 millicuries of several radionuclides was shipped.

Each container was labeled as Radioactive White.I or Yellow II.

Radiation survey indicated a maximum dose rate of 1.5 mR/hr at the surface of the trailer and less than 0.5 mR/hr at 6 feet from-the vehicle. Radiation measurement in the cab was less than 2 mR/hr.

Sampling wipes of the trailer and containers indicated no removable ' , radioactive contamination.

During the unloading, the Eberline Model E'520 with the side window GM detector was used-to survey the container for conformance with the DOT labels. The U.S. Geological Survey container marked "GS-3" had a radiation measurement of 80 mR/hr at the surface of the container, and 2 mR/hr at 3 feet from the container.

The entries on the Radioactive

Yellow II labels were Na-24, 6 mci and transportation index of 0.8.

Radiation measurement indicated a~ dose rate of 70 mR/hr at the surface of this container, and 2 mR/hr at 3 feet from the container.

The entries on the Radioactive Yellow II labels were Na-24, Sr-85, 1 millicurie, and 0.8 transportation index. A copy of the Radioactive Waste Shipment and Disposal Form is attached as Appendix A.

49 CFR 172.403 (c)(1) specifies that a Radioactive Yellow II labeled } package may not measure more than 50 millirem per hour at any point

on the surface of the container and not exceed 1 millirem per hour at

three feet from the external surface of the package.

Container GS-3 maximum surface' dose rate was measured to be 80 mR/hr, and 2 mR/hr at-3 feet from the container, and container GS-ll had a maximum surface - dose rate of 70 mR/hr, and 2 mR/hr at 3 feet from the container.

Since these containers were labeled with Radioactive Yellow II and the radiation levels of these containers exceeded the regulations, it appears that the i Radioactive Yellow II labels were improper for containers GS-3 and GS-ll from the U.S. Geological Survey.

The representative of the State of Washington informed the inspector that the shipper would receive a warning letter concerning this item of noncompliance.
