The Attachment, the Steam Generator Tube Inspection Report, is being submitted in accordance with Wolf Creek Generating Station (WCGS) Technical Specification (TS) 5.6.10, "Steam Generator Tube Inspection Report." This report provides the results of WCGS's seventeenth steam generator (SG) tube inservice inspection. The Steam Generator Tube Inspection Report includes the following:

a. The scope of inspections performed on each SG;
b. Active degradation mechanisms found;
c. Nondestructive examination techniques utilized for each degradation mechanism;
d. Location, orientation (if linear), and measured sizes (if available) of service induced indications;
e. Number of tubes plugged during the inspection outage for each active degradation mechanism;
f. Total number and percentage of tubes plugged to date;
g. The results of condition monitoring, including the results of tube pulls and in-situ testing; The following items are also included in accordance with the One-time Alternate Repair Criteria, H*, which was approved by the Nuclear Regulatory Commission (NRC) to support WCGS's Refueling Outage 18 and the subsequent operating cycle:
h. Following completion of an inspection performed in Refueling Outage 18 (and any inspections performed in the subsequent operating cycle) the primary to secondary LEAKAGE rate observed in each SG (if it is not practical to assign the LEAKAGE to an individual SG, the entire primary to secondary LEAKAGE should be conservatively assumed to be from one SG) during the cycle preceding the inspection which is the subject of the report;
i. Following completion of an inspection performed in Refueling Outage 18 (and any inspections performed in the subsequent operating cycle) the calculated accident induced leakage rate from the portion of the tubes below 15.2 inches from the top of the tubesheet for the most limiting accident in the most limiting SG. In addition, if the calculated accident induced leakage rate from the most limiting accident is less than 2.50 times the maximum operational primary to secondary leak rate, the report should describe how it was determined; and P.O. j. Following completion of an inspection performed in Refueling Outage 18 (and any inspections performed in the subsequent operating cycle) the results of monitoring for the tube axial displacement (slippage). If slippage is discovered, the implications of discovery and corrective action shall be provided.

Enclosures 1, 2, 3 and 4 contain all service-induced indications identified in SGs A, B, C and D during WCGS's Refueling Outage 18.

Attachment to ET 11-0008 Page 1 of 8 STEAM GENERATOR TUBE INSPECTION REPORT This report is being submitted pursuant to Technical Specification 5.6.10, "Steam Generator Tube Inspection Report," to report the results of the seventeenth steam generator (SG) tube inservice inspection.

S ummary The seventeenth SG tube inservice inspection was completed by Westinghouse Electric Corporation on April 12, 2011, during Refueling Outage 18 (RF18). Initial entry into MODE 4 was achieved on May 24, 2011.

Refueling Outage 18 was the third inspection of the third inspection interval (60 Effective Full Power Months (EFPMs)). At the Start of Refueling Outage 18 the SGs had operated for a total of 262.08 EFPMs (21.84 Effective Full Power Years (EFPYs)).

Permanent approval of an alternate repair criteria, H*, for indications within the tubesheet (TS) could not be achieved prior to Refueling Outage 18. As a result, a One-time Alternate Repair Criteria, H*, was approved by the Nuclear Regulatory commission (NRC) which revised Technical Specification 5.5.9, "Steam Generator (SG) Program" and Technical Specification 5.6.10, "Steam Generator Tube Inspection Report" for Wolf Creek Generating Station's (WCGS's) Refueling Outage 18 and the subsequent operating cycle. The following alternate tube repair criteria shall be applied as an alternative to the 40% through wall depth (TWD)-

based criteria:

H* is the distance from the top of the tubesheet (TTS) into the TS expansion region that tubes must be free of stress corrosion flaws to assure that the tubes do not pull out of the TS under the limiting loads with appropriate factors of safety. For WCGS, the H* distance is 15.2 inches.

All defects below the H* distance are not relevant to the integrity of the tube. Therefore inspection and repair below the H* distance is not required by the technical specification. For Refueling Outage 18 and the subsequent operating cycle, tubes with service-induced flaws located greater than 15.2 inches below the TTS do not require plugging. Tubes with service-induced flaws located in the portion of the tube from the TTS to 15.2 inches below the TTS shall be plugged upon detection.

No corrosion related tube degradation mechanisms were detected in any region of the SGs.

Wear at anti-vibration bar (AVB) intersections; wear at the flow distribution baffle; and wear due to foreign objects continue to be monitored.

Secondary side maintenance and inspections were also performed, The condition of the SGs has met all industry and regulatory structural and leakage integrity requirements.

Section 1 of this report describes the scope of inspections performed, including the nondestructive examination techniques utilized for each degradation mechanism.

Section 2 contains the location, orientation (if linear), and measured sizes (if available) of service induced indications.

a. Active degradation mechanisms found;
b. Number of tubes plugged during the inspection outage for each active degradation mechanism;
c. Total number and percentage of tubes plugged to date;
d. The results of condition monitoring, including the results of tube pulls and in-situ testing;
e. The primary to secondary leakage rate observed in each SG;
f. The calculated accident leakage rate from the portion of the tube below 15.2 inches from the top of the tubesheet for the most limiting accident in the most limiting SG; and
g. The results of monitoring for the tube axial displacement (slippage).

Attachment to ET 11-0008 Page 3 of 8 STEAM GENERATOR TUBE INSPECTION REPORT Section 1: Scope and Examination Techniques of Inspections Performed The actual inspections performed during Refueling Outage 18 meet or exceed both the Technical Specification minimum requirements and the requirements of the Electric Power Research Institute (EPRI) Pressurized Water Reactor (PWR) Steam Generator Examination Guidelines. The following primary side inspections were performed in all four SGs:

  • 25% Bobbin examination of tubes, full length except for Rows 1 and 2, which are inspected with the bobbin from tube end to tube support plate (TSP) #7 on both the hot leg (HL) and cold leg (CL).
  • 25% HL +Point rotating pancake coil (RPC) examination, tube sheet (TS) +3"/-15.2".

0 HL RPC TS +3"/-1 5.2" of the peripheral tubes, 2 pitches into the bundle.

  • CL Peripheral Tubes, CL tube sheet (TSC) +/- 3" 100% of peripheral tubes; 2 pitches into the bundle.
  • +Point examination of all "I-code" indications that are not resolved after history review.

0 +Point inspection of new flow distribution baffle (FDB) wear indications and prior FDB wear indications that have changed by 10% TWD or greater by bobbin.

  • Visual inspection of mechanical and weld plugs.
  • +Point inspection to bound (all surrounding tubes, at least 1 pitch removed) the tubes exhibiting possible loose parts (PLP) signals during the inspection.
  • +Point inspection of PLP signals from the previous inspection.
  • 25% Row 1 and Row 2 U-bends, mid-range +Point examination.
  • Dents (structures) -Ž2volts: Inspect 25% in SG A/D and 50% in SG B/C of all previously identified and 100% of new dents a-2 volts in the HL (including the U-bends) with the mid-range +Point probe in all four SG's.

0 Dings (freespan) >5 volts: Inspect 25% of all previously identified and 100% of new dents >5 volts in the HL (including the U-bends) with the mid-range +Point probe in all four SG's.

  • 100% Bobbin inspection of all prior indications except dents and dings.
  • +Point examination of a 5% sample of bobbin indications that have not changed since the prior inspection ("H" and "S" codes).

0 +Point inspection of the sample of tubes to support the scale profiling effort.

  • 100% Bobbin inspection of tubes surrounding previously plugged tubes being monitored for potential long term damage propagation and tubes with potentially elevated residual stress.

Attachment to ET 11-0008 Page 4 of 8 STEAM GENERATOR TUBE INSPECTION REPORT In order to increase the probability of detecting foreign object wear, the base bobbin scope and TTS RPC was performed on different tubes. Auto Analysis (ADS) was performed as a tertiary review to the normal analysis process, utilizing sorts established to increase the confidence that top of tubesheet defects (DTI), PLP, indications of tube sever within the tubesheet (TSI),

support defects (DSI) and other significant flaws (>30% TWD) were detected. The sorts used were qualified against a data set made up of previous plant indications and indications that were "injected" into WCGS data by Data Union software. The tertiary review did not identify any indications that were not already called by primary and/or secondary analysis.

During the inspection, a C-2 finding for AVB wear in SG B led to a scope expansion. This was due to a single defective tube (45%) that had a growth rate of 25% over two cycles. An additional 20% of the unscheduled tubes in rows >15 were inspected. No further scope expansion was required.

In addition, the following secondary side maintenance and inspections were performed:

  • Sludge Lancing of all four SGs

" Foreign Object Search and Retrieval (FOSAR) to locate, identify, and retrieve foreign objects that were present on the steam generator TS in all four SGs. FOSAR was also performed to inspect for any PLP identified during the Eddy Current Program.

  • TTS In-Bundle Inspection (IBI) of SGs B and C to inspect the condition of the top of the TS and to augment the FOSAR effort.
  • Upper Steam Drum Inspection in SGs B and C to evaluate the condition of the upper steam drum components with regard to damage of any kind.

o 100% visual inspection of J-nozzles o Ultrasonic sample of feedring locations and J-nozzles Section 2: Indication Listing The following Enclosures contain all service-induced indications for all four SGs. Attachment to ET 11-0008 Page 6 of 8 STEAM GENERATOR TUBE INSPECTION REPORT Section 3: Inspection Results Active Degradation Mechanisms Found Note: "Active" degradation mechanism is a historical term (prior to the current Revision 7 of the EPRI PWR Steam Generator Examination Guidelines). This term has been replaced with "Existing" degradation mechanism. Existing Degradation is defined as indications of degradation previously and/or currently observed in a Steam Generator.

The inspection results indicate that wear at AVB locations, wear at the flow distribution baffle, and wear due to foreign objects are existing degradation mechanisms.

Stress Corrosion Cracking at the tube ends is also considered an existing degradation mechanism. However, an alternate repair criterion, H*, was approved for RF18. This eliminated the need to inspect the tubes below 15.2 inches from the top of the tubesheet.

  • An additional 3 plugs are installed in SG A cold leg only, due to tubesheet drilling errors during manufacturing. No tubes are installed in those locations.

Results of Condition Monitorinq The examination results confirm that the WCGS SGs currently meet all industry and regulatory structural and leakage integrity guidance. No indications were found to exceed structural limits.

No tube pulls or in-situ testing were required.

" AVB wear remains an existing degradation mechanism.

  • The maximum observed indication (45% TWD at an AVB location) meets the structural and leakage requirements.

" Tube wear at the FDB is an existing degradation mechanism that is not progressing.

Wear at the FDB is attributed to a prior application of pressure pulse cleaning of the SGs. No new indications were reported as expected; all indications were previously reported at Refuel 16 and Refuel 17. The magnitude of the wear does not challenge the performance requirements for condition monitoring.

  • Tube wear detected at three TSP intersections in SG D is an existing degradation mechanism that is not progressing. This degradation mechanism is not an operational degradation mechanism but is the result of transient foreign objects interacting with the tubes. The current indications of wear at the tube support plates meet the performance criteria for condition monitoring.
  • Volumetric indications detected at the TTS have not changed since the prior inspections at Refuel 16 and Refuel 17. The reported changes in depth are small and within the normal variability of the sizing technique. The likely source of these indications is wear due a transient foreign object at a prior time. No new wear indications from foreign objects were detected. No foreign objects were found among the tubes with volumetric indications and their neighboring tubes. The largest indication is 26% TWD, which poses no challenge to the condition monitoring performance criteria.

" No corrosion related degradation mechanisms were observed.

  • No degradation indications were observed related to potential precursor signals such as dents and dings, or to interferences such as permeability variations, etc.

" Leakage is not an issue for any indication reported. There is currently no leakage in the SGs.

  • All plugs were found to be in acceptable condition based on visual inspection.

" No wear was found on any tube adjacent to a tube that had previously been identified as potentially requiring stabilization due to continuing wear after plugging for AVB wear.

The absence of wear on adjacent tubes indicates that continuing wear of the plugged

Attachment to ET 11-0008 Page 8 of 8 STEAM GENERATOR TUBE INSPECTION REPORT tubes is progressing at a slower rate than predicted, confirming the analysis that significant margin in time exists before potential stabilization would be required.

" The accident induced leakage requirements for H* were met without the need for a change in the administrative leakage limit.

  • No evidence of tube slippage was found.
  • Sludge lancing activities removed 26 pounds from SG A, 34 pounds from SG B, 30 pounds from SG C, and 27.5 pounds from SG D.
  • FOSAR activities in SGs A, B, C, and D identified and removed some minor foreign objects. All foreign objects were bound using the Plus Point probe. In addition, PLP indications reported by eddy current testing were further evaluated by visual inspection and bounded using the Plus Point probe. No degradation was associated with any foreign object or PLP indication. Minor foreign objects remaining in the SGs were evaluated by analysis and are acceptable for continued operation.
  • Secondary side visual inspection activities performed at the top of the tubesheet in SGs B and C were successful in gathering additional information which is used to determine future maintenance and cleaning strategies. No degradation was identified during this inspection.
  • Upper steam drum visual and ultrasonic inspections in SG B and C collected additional data used for monitoring and trending of upper steam drum components. Minor degradation is being monitored at a few J-Nozzle to Feedring interface locations. All results were within acceptance criteria. No other degradation was observed during these inspections.
  • Analysis of the data collected for scale profiling is on-going and is expected in January of 2012. No immediate concerns are expected from this analysis. Previous analysis shows that the majority of scale deposits are located in the U-bend regions and upper tube support plate bundle on the hot leg side. At tube support plate quatrefoil locations, the majority of the locations examined have been clean. This information is used in conjunction with visual data to continue trending the condition of the secondary side of the steam generators in order to plan future maintenance and cleaning strategies.
