The attached Steam Generator Tube Inspection Report is submitted in accordance with Wolf Creek Generating Station Technical Specification 5.6.10.b. This report provides the results of Wolf Creek Generating Station's eleventh steam generator tube inservice inspection. The Steam Generator Tube Inspection Report includes the number and extent of tubes inspected, location and percent of wall-thickness penetration for each indication of an imperfection, and identification of tubes plugged. Attachment I provides the Steam Generator Tube Inspection Report.

Attachment I to ET 02-0046 Page 1 of 34 STEAM GENERATOR TUBE INSPECTION REPORT This report is being submitted pursuant to Technical Specification 5.6.10.b to report the results of the eleventh steam generator tube inservice inspection. Additional information established by NEI 97-06, "Steam Generator Program Guidelines," is also included with this report.

Summary The eleventh steam generator tube inservice inspection was completed by Westinghouse Electric Corporation on April 8, 2002, during Refuel 12. Section 1 of this report describes the inspection programs, which were completed in Steam Generators "B" and "C" including: 100%

bobbin coil except the U-bend portion of row 1 and row 2, 55% Rotating Pancake Coil (RPC) top of tube sheet on the hot leg side, 50% RPC row I and 2 U-bends, and a special interest program. The inspection results, which include the location and percent of wall-thickness penetration for each indication of an imperfection, are listed in Section 2 of this report.

Mechanical plugging was necessary due to indications found during the inservice inspection.

Of the 5604 tubes inspected in Steam Generator "B", five tubes were plugged due to anti vibration bar (AVB) wear indications that exceeded the 40 percent through wall Technical Specification acceptance criteria. One additional tube in Steam Generator B was administratively plugged due to a single circumferential anomaly. This tube was inspected with an ultrasonic technique, which confirmed that no crack-like indications were present. Three of the 5610 tubes inspected in Steam Generator "C" were plugged due to AVB wear indications that exceeded the 40 percent through wall Technical Specification acceptance criteria.

Identification of tubes plugged, as well as the total number and percentage of tubes plugged to date for each steam generator, is included in Section 3 of this report.

The results of the eleventh steam generator tube inservice inspection have been classified as Category C-2. Category C-2 is defined in Technical Specification 5.5.9 as inspection results that indicate "one or more tubes, but not more than 1% of the total tubes inspected are defective, or between 5% and 10% of the total tubes inspected are degraded tubes." Steam Generator "B" had 0.09 percent defective tubes and 2.64 percent degraded tubes. Steam Generator "C" had 0.05 percent defective tubes and 1.57 percent degraded tubes.

All freespan indications reported were reviewed in history. Those indications, which could either not be detected or showed a change from history, were reported as an "I"code and RPC tested. A sampling of calls that were found not to have changed from history were also RPC tested. RPC inspections were also performed to track Flow Distribution Baffle wear indications. None of the freespan or support plate indications tested with RPC were reported as degradation.

In addition, two tubes in Steam Generator B were tested with ultrasonic examination. No crack like indications were observed in the inspected region with the shear wave transducers.

Section 2: Location and Percent of Wall Thickness Penetration for Each Indication of an Imperfection.

The following tables contain all tubes inspected that had a percent through wall loss indication.

In addition, tube (55,71) in Steam Generator B contained a single circumferential anomaly.

This tube was further inspected with an ultrasonic technique, which confirmed that no crack-like indications were present. This tube was administratively plugged.

Attachment I to ET 02-0046 Page 34 of 34 STEAM GENERATOR TUBE INSPECTION REPORT Summary of Tube Integrity Assessment The inspection results confirm that the Wolf Creek Steam Generators currently meet all industry and regulatory structural and leakage integrity guidance.

The operational degradation mechanisms observed during this inspection were tube wear at the AVB intersections and a single indication of wear at a support plate intersection. A number of new AVB wear indications were found in both Steam Generators "B" and "C". AVB wear in Steam Generators "B" and "C"meets the definition of an active degradation mechanism. Tube wear at the support plates does not meet the definition of an active degradation mechanism, but should be regarded as a relevant (i.e., observed but not active) mechanism.

All structural and leakage criteria are projected to be satisfied for Steam Generators "B" and "C" through, at least, end of cycle 14, when they will next be inspected. Based on the results of this inspection, Steam Generators "A" and "D", which were not inspected during Refuel 12, are projected to satisfy all structural and leakage criteria through, at least, end of cycle 13. Steam Generators "A" and "D"will be next inspected at the end of cycle 13.
