U1 PTI-90-1, 2 and 9 test deficiencies notices (TON) and change notices (CN) were reviewed. All issues associated with the Steam Generator Slowdown monitors were in PTI-90-09. There were 4 TON's and 1 CN.

TON 94-0843 was an inadequate sample flow issue related to a flow orifice in the sample line with a 0.125 inch 10. The resolution was to increase the orifice size to 0.87 inches. In a later OCN during U1 operation that orifice was replaced with one that was 0.2800 inches in conjunction with pressure control valve 1-PCV-90-120 replacement. For U2 the same model PCV valve is used with a new orifice installed with an 10 of 0.2693 inches. The minimum sample flow rate will be verified as part of this PTI.

TON 94-0948 was air bubbles appearing in the glass rotometer and an inadequate sample cooling issue. The air bubbles was resolved through a work order as a maintenance issue and the U2 2-RE-90-120/121 monitors design does not contain a rotometer. For the inadequate sample cooling it was determined that the sample line was connected tubing side of the heat exchanger 1-HTX-90-120 when it should have been connected to the shell side.

OCN 33253 corrected this issue for U1 and from drawing 2-47W600-106A this has been corrected for U2.

This was resolved on U2 by 0-SOI-24.01 Attachment 2V.(2) Added step to allow for turning the sample pump off prior to performing valve line-up. In this PTI the valve line-up is performed prior to pump start and prior to Hot Functional Testing.

Therefore all applicable Test Deficiencies and Change Notices from U1 pre-op testing have been incorporated in 2-PTI-90-05.

TDN-95-0797 discusses air bubbles were observed in the local flow indicators when the test was attempted at the 250 °F plateau. The resolution was to perform the test at the 350°F plateau. The HFT portion of this U2 PTI is defined and scheduled for the 557°F plateau so this should not be a problem.

TON 94-0773 had a step that failed when the green operate lighted pushbutton was able to be reset during a condition when it was not expected to reset. The resolution was the procedure was incorrect and that was the RP-30AM module worked as it was designed.

Additionally the RM-80 model and not the RP-30AM is used for 2-RM-90-120/121.

1.0 INTRODUCTION

1.1 Test Objectives Demonstrate the capability of the 2-RM-90-120/121 Steam Generator Slowdown Radiation Monitors to continuously monitor the common Steam Generator Slowdown liquid effluent, record radiation levels, divert blowdown from cooling tower blowdown to the condensate demineralizer system upon exceeding the high radiation setpoint and initiate radiation/ malfunction alarms.

Standard precautions shall be followed for working around energized electrical equipment in accordance with TVA Safety Manual Procedure 1021.

B. Steps may be repeated if all components cannot be tested in a step. However, if the test has been exited, prerequisite steps must be re-verified and a Chronological Test Log (CTL) entry made.

C. Discrepancies between component ID tags and description in a procedure/instruction do not require a Test Deficiency Notice, TON, in accordance with SMP-14.0, if the UN IDs match, exclusive of place keeping zeros and train designators (e.g.; 2-HS-31-468 vs. 2-HS-031-0468) and the noun description is sufficient to identify the component. If the component label needs to be changed, a Tag Request Form (TR Card) should be processed in accordance with Tl-12.14. Make an entry in the CTL and continue testing.

D. All wires removed/lifted from a terminal shall be identified and taped or covered with an insulator to prevent personnel or equipment hazard and possible spurious initiations. MAI-3.3 Data Sheet 1 shall be used for lifted wires. The wires should be grouped together and labeled with the work implementing document number that required them to be lifted if left unattended.

E. All open problems are to be tracked by a corrective action document and entered on the appropriate system punch list.

F. Observe all Radiation Protection (RP) requirements when working in or near radiological areas.

G. Ensure there are no adverse effects to the operation of Unit 1structures, systems, or components.

H. Test personnel will coordinate with Unit 1 Operations when manipulating Unit 1 equipment if required.

I. During the performance of this procedure visual observation of piping and components is required. This includes steady state and transient operations with visual confirmation that vibration is not excessive.

J. If the vibration is determined to be excessive the Test Engineer shall initiate a Test Deficiency Notice (TON).

K. When the RM-23P is connected to RM-80, communications with the MCR RM-23A will be disconnected. Therefore current indications for the MCR RM-23A, computer points and control room recorder will not work with the RM-23P connected.

L. RM-23P connections should be made prior to applying power to the RM-23P and power should be removed from the RM-23P prior to disconnecting it.

M. When handling a circuit board, a wrist strap that is grounded to rack chassis ground must be worn to prevent damage to the board due to static electricity.

N. Removing or installing any printed circuit board, when RM-80 is energized may cause damage to printed circuit board.

0. When installing or removing printed circuit boards, ensure that slot keys remain in proper slots (or replace when pulled).

P. Operations assistance will be required to acknowledge and reset annunciators in Control Room.

Q. A portion of this test, Section 6.15 is to be performed during the Unit 2 Hot Functional Testing. There will be hot piping and potential steam leaks that can present a new hazardous environment compared to when other testing on this equipment was performed.

R. 2-FCV-15-44 is a U1/U2 Boundary Device controlled by Tl-12.08, Control of Unit Interfaces.

S. 120 VAC Power plug for 2-RR-90-120 in Main Control Room Panel 0-M-12 is a Boundary Device controlled by Tl-12.08, Control of Unit Interfaces.

T. Radiological Worker and radioactive source handling training is required for handling radioactive sources.

U. Use of radioactive sources shall be coordinated with Radiological Control.

V. On each Attachment where M&TE is used there is a table where the M& TE that was used should be filled out so that if any M&TE out of calibration reports are generated it can be easily determined where that instrument was used.

4.1 Preliminary Actions

[1] EVALUATE open items in Watts Bar Integrated Task Equipment List (WITEL), AND ENSURE they will NOT adversely affect the test performance and results.

[2] ENSURE changes to the references listed on Appendix B have been reviewed, and determined NOT to adversely affect the test performance.

[3] VERIFY current revisions and change paper for referenced drawings has been reviewed and determined NOT to adversely affect the test performance, AND ATTACH documentation of current drawing revision numbers and change papers that were reviewed to the data package.

[4] VERIFY the test/performance copy of this Preoperational Test Instruction (PTI) is the current revision including any change notices and as needed, each test person assisting in this test has the current revision including any change notices.

6.15.1 Hot Functional Testing Setup NOTES

1) Coordinate with Operations Personnel and the system 15 Startup Test engineer so that testing between system 15 (Steam Generator Slowdown) and system 90 do not conflict.
2) 2-THV-24-1 000 may need to be throttled to provide optimum cooling to 2-HTX-90-120.

[1] ENSURE the following systems are in service and aligned to the extent necessary to perform this test A. System 15, Steam Generator Slowdown System is available at normal operating temperature and pressure and valve 2-FCV-15-44 is energized and available to support testing of 2-RE-90-120 and 2-RE-90-121 .

B. System 24 raw Cooling water System is available to supply cooling water to 2-HTX-90-120.

[2] ENSURE the as left position of each of the valves associated with 2-RE-90-120 and 2-RE-90-121 skids are in accordance with the desired position stated in Attachment 16, AND RECORD that position the as left position in Attachment 16.

[3] OPEN valve 2-ISIV-90-120K, External sample outlet isolation fully open.

2) The normal temperature operating range of the radiation monitor detector is 40 of to 120 °f. Therefore while opening the 2-ISV-90-120 for the first time the 2-ISIV-90-120A temperature should be monitored to ensure temperature limitation is not exceeded.

[7] PLACE the SGBD monitor in service and provide water to it per SOI-90.01 or equivalent approved TOP or SOl for this purpose.

[5] ADJUST valve 2-ISIV-90-120H UNTIL greater than 6.0 GPM is indicated on the calibrated flowmeter.

[6] CHECK DMM indicates 0 VDC (contacts closed) across the switch contacts.
