Title | Failure to Promptly Identify and Correct a Condition Adverse to Quality Associated with Operation of Essential Chillers |
Description | The inspectors identified a noncited violation of 10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, for the licensees failure to promptly identify and correct a condition adverse to quality associated with Unit 3 essential chiller A oil level. The condition adverse to quality involved inadequacies in plant operating procedures used to operate the chiller at low load conditions. As a result, the chiller was declared inoperable six times between July 27, 2011 and September 5, 2011 due to low oil level caused by oil migration into the refrigerant, a known phenomenon that occurs during low load conditions. The licensee implemented a temporary procedure change to ensure sufficient load is placed on the essential chiller during operation. The licensee entered the issue into the corrective action program as PVAR 3892184 and has not completed all corrective actions for the issue. The failure to promptly identify and correct a condition adverse to quality associated with operation of essential chillers was a performance deficiency. The performance deficiency is more than minor and is therefore a finding because it is associated with the equipment performance attribute of the Mitigating Systems Cornerstone and affects the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Inspection Manual Chapter 0609.04, Phase I Initial Screening and Characterization of Findings, the inspectors concluded the finding was of very low safety significance (Green) because the finding: (1) is not a design or qualification issue; (2) did not represent an actual loss of safety function of the system or train; (3) did not result in the loss of one or more trains of non-technical specification equipment; and (4) did not screen as potentially risk significant due to a seismic, flooding, or severe weather imitating event. The finding has a crosscutting aspect in the area of problem identification and resolution associated with the corrective action program component because the licensee failed to take timely corrective actions after identifying inadequacies in the operating procedure during review of similar issues that occurred in Unit 2 P.1(d)(Section 4OA2) |
