| Title | Failure to Analyze Refueling Water Storage Tank Level Transmitters for High-Energy Line Break |
| Description | The inspectors identified a noncited violation of 10 CFR Part 50, Appendix B, Criterion III, Design Control, for the licensees failure to adequately evaluate a potential high-energy line break in nonseismically qualified auxiliary steam piping in the refueling water storage tank valve house. The harsh environment from a high-energy line break had the potential to impact safety related level transmitters associated with the refueling water storage tank. Following identification of this issue by the inspectors, the licensee analyzed the nonnuclear auxiliary piping to ensure it could withstand safe shutdown earthquake loadings which allowed high-energy line breaks at intermediate locations to be excluded. This issue was entered into the licensees corrective action program as Callaway Action Request 201102588. This finding is greater than minor because it is associated with the Mitigating Systems Cornerstone attribute of design control and affects the associated cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Manual Chapter 0609.04, Phase 1 - Initial Screening and Characterization of Findings, this finding is determined to be of very low safety significance since subsequent evaluation concluded the issue was a design or qualification deficiency confirmed not to result in loss of operability or functionality. This finding did not have a cross-cutting aspect since the error associated with the high-energy line break analysis was not reflective of current licensee performance (Section 1R15). |
