Description | Title 10 CFR 50 Appendix B, Criterion XVI, Corrective Action, states, in part, that measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected. Contrary to these requirements, plant personnel failed to properly evaluate and correct a damaged temperature control valve on the Division II emergency diesel generator jacket water system. During maintenance on the valve, the valve bonnet assembly fell approximately three feet from the top of the work table to the concrete floor causing a bend in the valve crank frame, which is a non-pressure retaining part. A condition report documented the damage, but station management failed to perform a formal assessment or a use as-is evaluation before installing the damaged pressure control valve bonnet assembly back into the valve body on the Division II emergency diesel generator. The finding is considered to be of very low safety significance (Green), because it was not a design or qualification deficiency; did not represent either a loss of system safety function, an actual loss of safety function of a single train, or an actual loss of safety function; and did not screen as potentially risk significant due to a seismic, flooding, or severe weather initiating event. The issue has been entered into the licensees corrective action program as condition report CR-RBS-2010-04785. |
