Title

Failure to Plug a Main Condenser Tube in Accordance with an Approved Work Order

Description

The performance deficiency was more than minor because it was associated with the human performance attribute of the Initiating Events Cornerstone and affected the cornerstone objective of limiting the likelihood of those events that upset plant stability and challenge critical safety functions during shutdown, as well as power operations, in that the performance deficiency created a condition that upset plant stability by creating a condenser tube leak that prompted the plant to reduce power. The inspectors determined that the apparent cause of this finding was the licensees failure to use human performance error-prevention techniques to ensure that the tube plugging was performed correctly. This finding therefore has a crosscutting aspect in the work practices component of the human performance area because the licensee did not communicate and use human error prevention techniques commensurate with the risk of the assigned task, such that work activities are performed safely.
