Title | Failure to Make Timely Corrective Actions to Repair the Degraded Auxiliary Building Water Intrusion Barrier |
Description | The inspectors identified a non-cited violation of 10 CFR 50, Appendix B, Criterion XVI, Corrective Action, involving the failure to correct a condition adverse to quality in a timely manner. Specifically, the licensee failed to correct multiple degraded conditions associated with the auxiliary building water intrusion barrier. The licensee entered this issue into their corrective action program as Condition Report CR-GGN-2012-10314. Corrective actions included generating Work Order 318398 and delegating funds to repair the water intrusion barrier at the next available opportunity. The finding is more than minor because if left uncorrected, the condition of a degraded auxiliary building water intrusion barrier could lead to a more significant safety concern. Specifically, continued degradation of the water intrusion barrier could lead to the auxiliary building (secondary containment) being degraded such that the standby gas treatment system would not be able to achieve and maintain the design negative pressure of 14 inch water column within 120 seconds. Using Inspection Manual Chapter 0609, Attachment 4, Initial Characterization of Findings, the inspectors determined that the finding affected the Barrier Integrity Cornerstone. In accordance with Inspection Manual Chapter 0609, Appendix A, The Significance Determination Process (SDP) for Findings at Power, the inspectors determined that the finding had very low safety significance (Green) because the finding only represents a degradation of the radiological barrier function provided for the auxiliary building and standby gas treatment system. The inspectors determined that the apparent cause of this finding was that the licensee had failed to classify the degraded water intrusion barrier as a condition adverse to quality that warranted correction in a timely manner. Therefore, the finding has a cross-cutting aspect in the problem identification and resolution area, corrective action program component because the licensee failed to properly classify conditions adverse to quality |
