.Failure to Perform a Functional Test of the Power Range Neutron Flux Low Trip Setpoint when Greater than 25% Reactor Power CONDITIONS PRIOR TO OCCURRENCE:

Load Changes due to Routine Power Operations DESCRIPTION OF OCCURRENCE:

A supervisor in the Instruments and Controls Group identified the failure of our procedure to comply with the Technical Specifications

{3.3.1.1) by not testing the low trip setpoint when performed

greater than 25% reactor power.

Th~ design of the circuit for the power range neutron flux detector did not allow testing of setpoints lower than the power level being generated.

A test signal was inserted, adding to the power level indicated, to reach the desired level in order to perform the monthly functional test.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:.

The manufacturer's, Westinghouse, generic procedure to perform a functional test of the trip setpoints on the power range neutron flux recommends the method used by the Instruments and Controls Group to perform this test.

ANALYSIS OF OCCURRENCE:

The procedure used at our station was essentially a duplicate of the generic procedure provided by the manufacturer and the conditions overlooked by the manufacturer were also overlooked by our personnel.

This problem has* existed since initial licensing in 1976 until the start of our 1979 Refueling Outage.

During the outage, the procedure was* evaluated and corrected.

Since the outage, the monthly functional tests have been completed in accordance with Technical Specification 3.3.1.1.

LER ~80-02/03L CORRECTIVE ACTION:

An on-the-spot change was written to the functional test procedure which allow the detector to be removed from the circuit and a test signal is inserted to simulate all power levels.
