Operatonal Mode 6 -

Refueling DESCRIPTION OF OCCURRENCE:

Evaluation of No. 13 Component Cooling,Pump March surveillance perfor-mance data in accordance with ASME Code Section XI showed pump delta P in the alert range.

The pump was disassembled during the refueling outage and inspection of the impeller showed porosity and linear indications where the impeller vanes meet the outer ring.

It was decided to replace the impeller with an on-hand spare while waiting for evaluation of the indications.

Inspection of the spare impeller showed the same type of indications in the same approximate locations.

The two impellers were sent back to the manufacturer for evaluation of the indications while No. 11 and 12 Component Cooling Pumps were being disassembled.

Inspection of No. 11 and 12 pump impellers showed similar indications.

DESIGNATIO~ OF APPARENT CAUSE OF OCCURRENCE:

The impeller indications were the result of foundry, welding and cleaning techniques.

ANALYSIS OF OCCURRENCE:

Gould Pumps, Inc. has investigated the pump imperfections and their evaluation concludes that the indications, based on a metallurgical viewpoint, are not to be considered significant or result in propa-gation to effect fatigue life.

The pumps, including impellers, were manufactured prior to the advent of ASME Code requirements.

The impellers did not receive any visual inspection or other types of quality examination.

Public Service Engineering Department accepts the vendors evaluation to the extent that the pump impellers were in no eminent danger of failure due to the detected impeller indications.

LER 79-45/0lX-l CORRECTIVE ACTION:

The original pump impellers have been replaced with new, thoroughly inspected, stainless steel impellers and no further action is required.
