performing Surveillance Test ST 5074.01 on Component Cooling Water (CCW) '

Train 1 the operators discovered that CC1467 (Decay Heat Cooler 1-1 CCW Outlet Valve) would not open. Decay Heat Train I was declared inoperable,

and the unit entered the action statement of Technical Specification 3.5.2, which requires the inoperable train to be restored to operable ,

.j Designation of Apparent Cause of Occurrence: The apparent cause is a

! manufacturing error. On November 29, 1982, Instrument and Control.(I&C) mechanics found the actuator shaf t to be bent, and the actuator was bound up. Under Maintenance Work Order IC-661-82, the actuator was disassembled, inspected, and reassembled with a new actuator shaft installed. On.

November 30, 1982, engineering and station personnel inspected the valve actuator and by comparing it with the actuator for CC1496, Decay Heat Cooler 1-2 CCW Outlet Valve, discovered that a flanged bearing was missing from the actuator mounting plate. When the bearing was removed or lost cannot be determined. ,

Analysis of Occurrence _
There was no danger to the health and safety of
the publi
ur station persennel. Decay Heat Train 2 was operable and available to provide core cooling.

Corrective Action: CC1467 was placed in the open position with the air l 1sclated, and Decay Heat Train 1 was declared operable on November 30,

On December 15, 1982, a new flanged bearing was installed in CC1467 under Maintenanca Work Order IC-661-82. CC1467 was then tested per Surveillance Test ST 5074.01, Section 8, and declared j operable at 1540 hours0.0178 days <br />0.428 hours <br />0.00255 weeks <br />5.8597e-4 months <br /> on December 15, 1982. To insure that the flanged bearing is properly installed, Maintenance Instruction 11-803 has been written to provide maintenance guidelines on CC1467 and CC1469.

Failure Data
Previous failures of CC1467 to open have been reported in Licensee Event Reports NP-33-79-114 (79-098) and NP-33-79-145 (79-125).
