Title | Failure to establish appropriate MR performance goals |
Description | The inspectors identified a Green NCV of 10 CFR 50.65(a)(1) associated with Unit 1. Specifically, PSEG did not establish appropriate performance goals in response to positive displacement pump (PDP) performance issues that resulted in significant emergent unavailability and a repeat maintenance preventable function failure (RMPFF). PSEG entered this issue into their CAP to evaluate PDP performance goals and action plans. The finding was more than minor because it was associated with the Equipment Performance attribute of the Mitigating System cornerstone and affected its objective to ensure the availability and reliability of systems (safe shutdown charging cross-connect) that respond to initiating events (fire) to prevent undesirable consequences (i.e., core damage). The inspectors evaluated the finding in accordance with IMC 0609, Appendix F, Fire Protection Significance Determination Process. The inspectors determined that Finding Category 1.4.5 (post-fire, safe shutdown) applied as the finding potentially impacted a system credited for post-fire, safe shutdown. The inspectors determined that the finding was of very low safety significance (Green) because the Unit 2 reactor would have been able to reach and maintain safe shutdown, crediting the Unit 1 operating centrifugal charging pump as necessary (based on a yes response to question 1.3.1.A). This finding has a cross-cutting aspect in the area of Human Performance, Procedure Adherence, in that PSEG personnel did not follow Maintenance Rule (MR) processes and procedures. Specifically, PSEG personnel did not follow MR program procedure guidance to set appropriate (a)(1) monitoring goals or revise existing (a)(1) monitoring goals to monitor the effectiveness of actions taken to restore PDP performance. |
