5.0 PROCEDURE

5.1Originating RPAs-theoriginator oftherequisition orchangetotherequisition shall:l.includeinformation suchthattheitemrequested isdescribed inenoughdetailthatitisclearwhatisdesired.Depending onthecomplexity ofwhatisrequested, thedetailsmayconsistof:a.scope(extentofworktobeperformed orapplication ofitem).TheICCzC'copemaybeintheformofaspecification orcontractwhichshallbereferenced ontheRPAandifthesupplierdoesnothaveaccesstothesedocuments theymustbeattachedtotheRPA;b.referenced and/orattachedspecificdrawings, codes,standards, procedures, instructions, etc.,including theparticular revisiondata(wheretherevisioniscriticaltotheproperform,fitorfunction).

ThePlantQCGroupshallreviewthesourcedocuments (e.g.,Q-List,Spin-List, LineList,Instrument List,ValveList,Drawings, etc)whichidentifysafetyrelateditemsandrequirements asapplicable.

Theperformance anddocumentation ofthisreviewshallbedescribed inplantprocedures.

Ifqualityrequirements arerequired, thenQCshallprovidethenecessary qualityrequirements toassurethattheitemmeetsorexceedstherequirements oftheoriginalspecification.

Methodsusedfordetermining thatsparepartsmeetorexceedoriginalrequirements shallbedelineated inplantprocedures orinstructions.

5.9QualityAssurance Department reviewofRPAs-theQADepartment shallreviewRPAsasstatedinthisprocedure toassureadequateinclusion ofinformation specified inparagraphs 5.1through5.8ofthisprocedure.

5.10QualityRequirements

-ifQCreviewindicates thattherequisition needstobestamped"QC-Required" thespecificqualityrequirements shallbereferenced on,writtenon,orattachedtotheRPAutilizing theformshowninFigures4.1-1.AllRPAsforitemsandservicesclassified "QC-Required" shallstatethattheprovisions of10CFRPartZlapplytothepurchaseandshouldreference thelatestrevisionofFPLSpecialQualityAssurance DocumentSQAD-100Z.

OtherQualityrequirements whichmaybeincludedon"QC-Required" or"QL-2"RPAsarethefollowing:

1.Requirements forthesuppliertohaveadocumented QualityAssurance Programthatimplements theapplicable portionsof10CFR50,AppendixB,orANSIN45.2.

Ifthisisnotexpedient, QAandQCshallbecontacted bytheoriginator forconcurrence CCCDVIJ0VlCZwithinoneworkingdayofplacement oftheverbalorder.Intheeventthatthenecessary requirements werenotimposed,asupplemental purchaseorderwillbenegotiated including thesenecessary requirements.

ThePurchasing Agent/Contracts Agentplacingtheconfirming ordershallassurethatthesupplierisontheFPLASLwhenrequiredbythisprocedure.

Theoriginator oftherequestshallfollowallconfirming purchaseorderswithanRPAthatwillberoutedforreviewandapprovalinaccordance withthisprocedure.
