Submittal of Written Exam Results and Proposed Changes for Initial License Class 2022 Dear Mr. Kirkland Enclosed you will find the written examination materials that were administered on 2/23/2022 at Callaway Station.

An exam item analysis performed showed that Questions 15, 17, 29, 37, 60, 61, 63, 85, 90, and 92 were high miss questions (~6 missed for the RO section and ~3 for the SRO section). The results of the exam item analysis found no question flaws with Questions 15, 17, 29, 37, 60, 61, 63, 85, 90; course enhancement #804 was generated to evaluate reinforcing / incorporating these topics.

Furthermore, an exam review was held on 2/25/22 with the 2022 applicants which resulted in challenges to Questions 70 and 92.

The results of the analysis of Question 70 and 92 are as follows:

  • Question 70. The stem provided a situation where a fire occurred in the Service Building First Floor and asked "what should the fire brigade use to ventilate the area". FPP-ZZ-00007, Attachment 21, Pre-Plan/Fire Area #YD-1 Service Building First Floor, Section 5 describes the use of portable and / or fixed ventilation systems. The keyed answer of portable smoke ejectors is from the Note prior to Step 5.2 that states " NOTE - Caution should be taken in using recirc. systems to exhaust smoke. This could result in transferring smoke to other areas of building and actuation of detectors in areas remote from the fire." However, there are several exhaust systems listed in Step 5.1.b, such as CUB1007A-C, that do not recirculate air but simply exhausts air out of the rooftop for some locations on the first floor of the service building. (Reference print 8600-X-90265).

Recommendation: Accept two answers. Both pieces of equipment are complementary to each other and may be used in conjunction to address a fire. The keyed answer is from a note; however, either portable smoke ejectors and I or some of the installed building HVAC units would be applicable equipment to exhaust smoke without introducing it into other areas of the service building. Since the stem did not specify a fire location in the service building first floor, both portable smoke ejectors and installed building HVAC systems are correct answers. This results in 2 correct answers: A and B.

A procedure change request, CR 202201188, was generated to clarify equipment use and revise Attachment 21.

  • Question 92. The stem provided a situation where SDRE0035 alarms above its HIHI alarm setpoint. The keyed answer is from Annunciator 62A, Area Radiation High High, step 3.4.1 which states "IF the affected area is accessible by plant personnel:

ANNOUNCE the following twice on the Gaitronics system: ... The (location) is off limits to all personnel until further notice." However, SDRE0035 in HIHI alarm is also an entry condition into OTO-KE-00001, Fuel Handling Accident. OTO-KE-00001, step #3 RNO to "Manually Actuate FBIS" is the first action the crew would take utilizing this procedure.

Recommendation: Accept two answers. The question stem did not provide a procedural condition of "Per Annunciator 62A ... ". Additionally, as both procedures could be implemented in response to the HIHI radiation indication, both actions are complementary and are focused on protecting onsite personal from radiation exposure.

Therefore, both actions for part 1 of answers A and Care correct, which would result in 2 correct answers: A and C.

The issues associated with Questions 70 and 92 were not recognized during exam development or validation.
