Dear Mr. Davis Reference (a) Section 3.3.3.6, Action C requires Commonwealth Edison to submit a Special Report, pursuant to Specification 6.9.2, within j fourteen (14) days that provides actions taken, cause of the inoperability and plans and schedule for restoring the Accident Monitoring Instrumentation channels to OPERABLE status. This is required in the event the subject instrumentation is inoperable for greater than seven (7) days.

Attachment (A) provides this report. I please direct any questions concerning this matter to this office.

i DESCRIPTION On June 14, 1987, at 1826, Wide Range Gas Monitor (WRGM) failed an 1 automatic check source test on its high range detector, IPC330. J The check source test is a self-contained function to varify /

detector operability. Since this test is the only approved method of determining detector operability, the WRGM was declared inoperable. The 7-day allowed outage time expired on June 21, 1987.

CAUSE The check source for IPC330 had become loose and fallen off. While attempting to repair it, the assembly containing the check source was damaged. The integrity of the check source itself was not compromised. A new check source assembly has been ordered from the vendor.

COMPLETION DATE Parts delivery and installation of the new check source is expected to be completed by July 15, 1987. This date is contingent upon the receipt of parts as scheduled.
