-letter to transmit such a report summarizing the findings resulting from a recent inspection of the containment tendons. I On September 18,1997 the results of the chemical analysis for the grease  !

samples that were collecud from the lower grease cans on Braidwood Unit 2 Vertical Tendons V217 and V241 were received. The moisture content in the grease samples for both of these tendons was determined to exceed the maximum 10% by weight. The grease in Tendon V217 had a 13% moisture / g content and Tendon V241 had a 14% moisture content.

Attached is a summary report, the required engineering evaluation, and the corrective actions being taken to address this issue.

Special Report Descriotion of Abnormal Deoradation:

Vertical Tendons V217 and V241 were included in the 1997 containment tendon water and visual inspection scope. The subject tendons have a history of free water collecting in the lower grease can and are therefore are included in the periodic inspection scope. These inspections are performed to identify free water intrusion and to perform chemical analysis on the sheathing filler to ensure it continues to protect the tendons as designed. The1997 inspections were performed under Braidwcod NWR 97005G944 and per approved contractor procedures. The contractor employed to perform the inspections was Precision Surveillance ':orporation and the contractor employed to perform the chemical analysis of the grease was Suburban Laboratories, Inc.

The inspections were performed by a qualified Quality Control inspector and the chemical analysis was performed by qualified personnel. The degradation involved the identification of excessive moisture content in the grease samples. The ac 7ptance criteria is a maximum of 10% by weight.

The identified moisture contents were 13% in the grease sample for tendon V217 and 14% in the grease sample for tendon V241. Free water was collected from the lower grease cans for both tendons. Forty two (42) ounces was collected from tendon V217 and forty one (41) ounces was collected from tendon V241. The results of the chemical analysis for chloridss, r., o % sulfides, specific gravity, and reserve alkalinity met the acceptance criteria. The previous inspection that was performed in June of 1996 resulted in the identification of twelve (12) ounces of free water in tendon V217 and (40) ounces cf free water in V241. The moisture content in the grease did not exceed the acceptance criteria. The moisture content in the grease for tendon V217 was 0.8% in the grease for V217 and 8.4% in the grease for V241. The results of the chemical analysis for chlorides, nitrates, sulfides, specific gravity, and reserve alkalinity for the 1996 <

inspection also met the acceptance criteria.

The remainder of the inspection attributes were acceptable. No active corrosion was identified.

Level 111 corrosion was initially identified on the anchorage head on tendon V217 in 1987 and has remained unchanged since. Level ill corrosion is characterized as steel that is reddish brown in color and showing evidence of heavy corrusion and encrustation and visible signs of pitting in excess of 0.005" in depth, but not greater than 0.010" in depth. No reportable corrosion has been identified on tendon V241 since construction. No concrete damage was identified. The tolerance for reporting concrete cracking is any crack that exceeds 0.010" in width. The button heads were identified to be in good condition for both tendons. A wire was removed for physical testing from both tendons in 1992. The wires from both tendons met the physical test acceptance criteria and were reported to be in excellent condition. No voids in excess of 5% of the net duct volume were identified and all parts of the anchorage system we,a adequately covered by grease

Section C.8 of Proposed Revision 3 to Regulatory Guide 1.35, Reoortino to the Commission:

The reportable conditions of regulatory position C.7.1.3, C.7.1.4, C.7.3, or C.7.4 could indicate possible abnormal degradation of the containment structure ( a boundary designed to contain radioactive materials ). Any such condition should be reported to the Commission in accordance with the recommended reporting program of Regulatory Guide 1.16 " Reporting of Operating Information - Appendix A Technical Specifications."

Technical Specification 4.6.1.6.le specifies the requirements for verifying the operability of the sheathing filler grease.

4.6.1.6.1e Verifying the OPERABILITY of the sheathing filler grease by assuring:

1) No voids in excess of 5% of the net duct volume,
2) Minimum grease coverage exists for the different parts of the anchorage system, and.

J) The chemical properties of the filter material are within the tolerance limits as specified by the manufacturer.

Braidwood Technical Specification Bases section 3/4 6.1.6 sta:es, " The required Special Reports from any engineering evaluation of containment abnormalities shallinclude a description of the tendon condition, the cond'tlon of the cuncrete (especially at the tendon anchorage), the inspection procedure, the tolerance on cracking, the results of the engineering evaluation and the corrective actions taken."

. Corrective Actions Taken:

An Engineering Evaluation was performed on the Unit 2 Containment Structure. This evaluation determined that the Unit 2 containment structure remains capable of performing its intended design function. This evaluation is included in this report. These tendous were found to have free water present since 1985 and have been included in periodic water tendon inspections. Current and previous inspections since 1987 have reported no evidence of active corrosion on the tendon anchorages. The tendons have also been includsd in previous in Service inspections and were selected for_ wire removal, inspection, and testing during the 5th year ISI in 1992. The wires were tested for tensile strength and elongation and were found to be satisfactory. A lift off test was also performed for these tendons with acceptable results. The results indicated no active corrosion on the wires or tendon anchorage. The abnormal condition has been discussed with the grease manufacturer. The manufacturer stated the water content is not an indication of degradation of the grease. As long as the grease continues to coat the tendon components, it will

- provide the required corrosion protection. The greas;. was replaced in the lower grease cans on both tendons. Each tendon had the replacement grease installed from the top can to ensure the entire area is filled. The tendons were restored by grease replacement on July 7 and July 9,1997.

Operations Shift Supervision was made aware of the problem with the crease analysis results at approximately 0900 on September 18th,1997.

Braidwood Station will continue to inspect these tendons for water on an annual basis. The grease in the sheathing for both tendons will be replaced during the next water tendon inspection. The n

next inspection is scheduled to be performed in the summer or early fall of 1998.

Potential Causes of Deoradation:

The cause of this degradation is assumed to be ground water intrusion. The top grease cans have been examined. There is no sign of grease leakage or water intrusion leak paths. The Braidwood Engineering Department will continue to evaluate the conditioti to determine if the exact source and pathway of the free water can be determined and corrected.

We have reviewed the grease test osta for Tendons V217 and V241 in the report prepared by Suburban

' Laboratories, loc. and submitted to us under Cc nEd NDIT # BRW DIT.97 327. The test data Indicates that the amount of watersoluble chloride, nittete and sulfide meet the requirements of Nuclear Regulatory Commission (NRC) Guide 1.35 (less than 10 ppm). However, the test data reflects moisture content of 13.0% (V217) and i

14.0% (V241) which are greater than the 10% maximum acceptance limit established tr/ NRC Regulatory -

, Guide 1.35.

l These tendons were found to have water present since 1985 and have been included in the periodic water j tendort inspections. Previous visualinspections have reported no evidence of active corrosim on the tendon

anchorages.

These tendons have also been included in previous in Service inspections (Isis) and were selected for wire removal, inspedian and teshn0 dunn0 the 5th year ISl in 1992. The wires were tested for tensile strengtn and elongation and were found to be satisfactory. A lift-off test was also performed for these tendons with acceptable results. The results of these Isis indicate that there was no active corrosion on the wires or tendon anchorages.

l These inspection results indlcate to us that the grease continues to perform its intended function re0ardless of j the higher moisture levelin the grease samples.

k We have a:so previously discussed a similar siteo'Jon with Mr. C. Novak of Viscosity Oil, the sheathina grease manufacturer. He noted that the analysis results indicate greaterwater contents as a result of the sample j

being taken at fM bottom of the tendon where water accumulatas. Therefore, it does not represent the effectn*eness of grease to protect the tendon. He also stated that the water content is not an indication of d*;Jradation of the grease. As long as the grease continues to coat the tendon and andlorage components,it j will provide corrosion protection.

Based on our engineering evaluation of the moisture content in the grease ramples for Tendons V217 and V241 and the inspection history to date for both of these tendons, the following is recommended:

1. Continue yearly watersurveillance for these tendons.
2. Replace the grease in the shes'.hin0 for both tendons durin0 the next water tendon inspection.
