ATTACHMENTTOPLA-3933FILER41-2PAGE7OF103.DrawingA-107,Detail4,Eley.,providesdetailsofaonehourgypsumfirebarrierenclosurewithanairgapbetweentheinnerandoutergypsumboard.Contrarytotheabove,asofSeptember4,1992,inFireZone0-28B-II,aone-hourratedelectricalbreakerenclosurefabricatedwithgypsumboarddidnothavetheairgapbetweentheinnerandoutergypsumboardmakingupthefirebarrierenclosureasspecifiedbytheinstallationdrawing,A-107,indrawingdetailnumber4.4.AdditionalexampleswhereSSES'srequiredfireprotectionqualityassuranceinspectionsfailedtoidentifyinstallationdeficienciesthatwerenotinconformance-tothedesignspecificationsaredetailedinSection4.3.4andAttachment3ofRegionIInspectionReport50-387&,388/92-23andareconsideredpartofthisviolation.~Resossse1.ReasonortheViolationThisviolationincorporatesnoncompliancesassociatedwithinadequatesurveillanceprocedureacceptancecriteriaanddiscrepancieswithinitialconstructiondesigndocuments.Inadequatesurveillanceprocedureacceptancecriteria(seeViolationA)resultedinthefollowingnonconformancesgoingundetected:tomZetexinFireZone0-28H,andtheinterfacebetweentheracewaywrapandthewallwasnotsealed.ThegypsumboardconstructiondeficiencyinFireZone0-28B-IIwouldonlyhavebeenidentifiedanddispositionedatthetimeofconstruction.InitialconstructionoffirebarriersattheSusquehannaSESwerenotquality-relatedactivitiesandinspectionsofworkactivitiesweredeterminedbyresponsiblefieldengineers.Norequirementexistedstatingthatallactivitiesbeinspected.Additionally,documentationassociatedwithconstructionofnon-qualityinspectionactivitieswasonlyrequiredtobemaintaineduntilsystemturnovertoPP&L.Nodocumentationonthegypsumboarddeviationwaslocated,thereforethereasonforthisviolationcannotbedetermined.,Inanycasethisdeficiencywouldnothavebeenidentifiedthroughnormalsurveillanceactivities.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieved:Acomprehensivewalkdownoffireratedracewaywrapinaccessibleareaswasconducted(seeViolationB).Resultsofthesewalkdownsidentifiedthreeracewayswithincorrectmaterials,approximatelyadozenminordiscrepancies,approximatelytwentyminormaintenanceconcerns(wearandtearitems),andvariousdrawingdiscrepancies.ThecompletiondateoftheactionsassociatedwiththesedeficienciesisestimatedtobeApril30,199
