ATTACHMENTTOPLA-3933FILER41-2PAGE4OF10~AdditionalexampleswherequalitywasinadequatelyappliedtothesetwoseriesofFireProtectiondesigndrawingsareidentifiedinSection5.1andAttachment3ofRegionIInspectionReport50-387and388/92-23andareconsideredpartofthisviolation.~Resense1.ReasonortheViolationTheerrorsintheReactorBuildingsandControlStructureE-294/295andC-1700seriesdrawings,utilizedforfireprotectionactivities,resultedfrominadequatedrawingverificationswhenthedrawingswereoriginallydeveloped.Followinginitialinstallationoffirebarriers(installedunderanonquality-relatedprogram),PP&LrequestedthatourA/Edevelopdrawingstoidentifythelocationoftheinstalledfirebarriers.Thesedrawings,E-294/295,wereneverfieldverifiedastofirebarrierlocationsortypeofmaterialutilized.TheC-1700seriesdrawingswhichidentifyracewaylayoutsweresubsequentlydevelopedutilizingtheE-294/295drawings.NofieldverificationswereperformedontheC-1700seriesdrawingsexceptforplantmodificationworkactivitiesthatutilizethesedrawings.Thesedrawingerrorswerethenpropagatedinsurveillanceprocedures(seeViolationA),engineeringstudiesandcalculationsthatutilizedinputsfromthesedrawings.Plantmodificationswerefieldverifiedpriortoinstallation.SeeViolation"C"foradditionaldeficienciesidentifiedfromthiswalkdown.2.CorrectiveStesWhichHaveBeenTakenandtheResultsWhenthedrawingerrorswereidentified,PP&Linitiatedacomprehensivewalkdownofplantareasthatcontainedfireratedracewaywrap.Allaccessibleracewayshavebeeninspected.Drawingerrorshavebeencorrected.Additionally,DrawingChangeNoticeshavebeenattachedtodrawingstoidentifytheactualfirebarriermaterialinstalledifmischaracterizedonthedrawings.Further,engineeringstudiesandcalculationswhichutilizedthesedrawingshavebeenreviewedandcorrected,wherenecessary.3.CorrectiveStesWhichWillBeTakentoAvoidFurtherViolations:a.TheappropriateclassificationoftheE-294/295andC-1700seriesdrawingsarebeingdispositionedandwillbemaintainedas"as-built"drawings.Additionally,byrevisionorattachment,thenotation"Quality-F"willbeaddedtothetitleblockofthesedrawings.TheestimatedcompletiondatefortheseactivitiesisApril30,1993.b.InaccessibleareawalkdownswillbecompletednolaterthanthestartupfollowingtheUnit2sixthrefuelingandinspectionoutagecurrentlyscheduledtoendonMay20,199 ATTACHMENTTOPLA-3933FILER41-2PAGE7OF103.DrawingA-107,Detail4,Eley.,providesdetailsofaonehourgypsumfirebarrierenclosurewithanairgapbetweentheinnerandoutergypsumboard.Contrarytotheabove,asofSeptember4,1992,inFireZone0-28B-II,aone-hourratedelectricalbreakerenclosurefabricatedwithgypsumboarddidnothavetheairgapbetweentheinnerandoutergypsumboardmakingupthefirebarrierenclosureasspecifiedbytheinstallationdrawing,A-107,indrawingdetailnumber4.4.AdditionalexampleswhereSSES'srequiredfireprotectionqualityassuranceinspectionsfailedtoidentifyinstallationdeficienciesthatwerenotinconformance-tothedesignspecificationsaredetailedinSection4.3.4andAttachment3ofRegionIInspectionReport50-387&,388/92-23andareconsideredpartofthisviolation.~Resossse1.ReasonortheViolationThisviolationincorporatesnoncompliancesassociatedwithinadequatesurveillanceprocedureacceptancecriteriaanddiscrepancieswithinitialconstructiondesigndocuments.Inadequatesurveillanceprocedureacceptancecriteria(seeViolationA)resultedinthefollowingnonconformancesgoingundetected:tomZetexinFireZone0-28H,andtheinterfacebetweentheracewaywrapandthewallwasnotsealed.ThegypsumboardconstructiondeficiencyinFireZone0-28B-IIwouldonlyhavebeenidentifiedanddispositionedatthetimeofconstruction.InitialconstructionoffirebarriersattheSusquehannaSESwerenotquality-relatedactivitiesandinspectionsofworkactivitiesweredeterminedbyresponsiblefieldengineers.Norequirementexistedstatingthatallactivitiesbeinspected.Additionally,documentationassociatedwithconstructionofnon-qualityinspectionactivitieswasonlyrequiredtobemaintaineduntilsystemturnovertoPP&L.Nodocumentationonthegypsumboarddeviationwaslocated,thereforethereasonforthisviolationcannotbedetermined.,Inanycasethisdeficiencywouldnothavebeenidentifiedthroughnormalsurveillanceactivities.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieved:Acomprehensivewalkdownoffireratedracewaywrapinaccessibleareaswasconducted(seeViolationB).Resultsofthesewalkdownsidentifiedthreeracewayswithincorrectmaterials,approximatelyadozenminordiscrepancies,approximatelytwentyminormaintenanceconcerns(wearandtearitems),andvariousdrawingdiscrepancies.ThecompletiondateoftheactionsassociatedwiththesedeficienciesisestimatedtobeApril30,199 ATTACHMENTTOPLA-3933FILER41-2PAGE9OF10C..ViolationD(387/92-23-04)TheFacilityOperatingLicense,NPF-14,wasissuedonJuly17,1982,forSusquehannaSteamElectricStationUnit1andcontainedacondition,2.C.(7),BatteryRoomArea[Section9.5.4,SafetyEvaluationReport(SER),SupplementalSafetyEvaluationReportSSER¹1andSSER¹3]statedthat:r"Priortoexceedingfivepercentof-fullpowerandsubjecttoNRCreviewandapproval,PP&Lshalleitherconductatanapproved'testinglaboratoryanASTME-119testoftheas-installedone-hourcablewrapconfigurationorinstallanautomaticfireextinguishingsystem."TheFacilityOperatingLicense,NPF-14wasamendedonMarch27,1990,byAmendment95toreviseLicenseCondition2.C.(6),whichstatesinpart:"PennsylvaniaPower&LightCompanyshallimplementandmaintainineffectallprovisionsoftheapprovedfireprotectionprogramasdescribedinthefireProtectionReviewReportforthefacilityandasapprovedintheSafetyEvaluationdatedAugust9,2989..."Contrarytotheabove,asofSeptember4,1992,SusquehannaSteamElectricStationwasfoundtohaveaKaowoolwrapfirebarrierinstalledinFireZone0-28H.,PP8rLdidnotsubjectthisKaowoolcablewraptoanASTME-119testatanapprovedtestinglaboratoryandPPALdidnotsubmitthisKaowoolcablewrapconfigurationtotheNRCforreviewandapprovalforuseinUnit1BatteryRoomArea,FireZone0-28H,attheSusquehannaSteamElectricStation.ThereisnoautomaticfireextinguishingsysteminstalledinUnit1BatteryRoomArea,FireZone0-28H.AdditionalexamplesofhowSSES'suseofKaowoolinFireZone0-28HdonotmeettherequirementsofDeviationRequest8or17oftheSusquehannaSteamElectricStationFireProtectionReviewReportaredocumentedinRegionIInspectionReport50-387/92-23andareconsideredpartofthisviolation.~Resonse1.ReasonortheViolationDeviationRequestNo.17oftheSusquehannaSteamElectricStationFireProtectionReviewReportrequestedadeviationfromtherequirementsof10CFR50,AppendixR,SectionIII.G.2.ctopermittheuseofKaowoolasaone-hourfirebarrierinplantareaswhereKaowoolwasalreadyinstalled.Thisdeviationrequestwasaconservativeactionintendedtodocumenttheplantconfiguration.KaowoolwasacceptedbytheNRCasan ATTACHMENTTOPLA-3933FILER41-2PAGE10OF10adequateonehourfirebarrierin,GenericLetter86-10whichwasissuedpriortorequestingthisdeviation.Thedeviationrequestlistedtheaffectedfireareas/zoneswhereKaowoolisinstalled.ThedeviationrequeststatedthatfirezonesthatcontainedKaowoolalsohadautomaticfiredetectionandsuppressioninstalled.PPEcLdidnotrecognizethatKaowoolwasinstalledinFireZone0-28HinDeviationRequestNo.17becauseofincorrectdrawingsusedtodeveloptheRequest(seeViolationB).DeviationRequestNo.8oftheSusquehannaSteamElectricStationFireProtectionReviewReportrequestedadeviationfromtherequirementsof10CFR50,AppendixR,SectionIII,G.2.atopermittheuseofaone-hourfirebarriertoseparateredundantsafeshutdownequipment/cablesinFireZone0-28B-I,0-28B-II,1-2D,and0-28Hwhenautomaticsuppressionisnotpresent.InrespondingtoDeviationRequestNo.8,NRCimplementedLicenseCondition2.C.7inOperatingLicenseNPF-14forSusquehannaUnit1whichrequiredPPEcLtoeitherconduct"atanapprovedtestinglaboratoryanASTME-119testoftheas-installedone-hourcablewrapconfigurationorinstallanautomaticfireextinguishingsystem."SincePPALdidnotrecognizethatKaowoolwasinstalledinFireZone0-28H,wedidnotprovideanASTME-119testasrequiredbythisLicenseCondition.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieved;TheKaowoolfirebarriersin0-28Hwerereplacedwithpre-formedThermo-Lagfirebarriermaterial.3.CorrectiveStesWhichWillBeTakentoAvoidFurtherViolations:EKaowoolfirebarriermaterialwillnotbeutilizedfornewfirebarrierinstallations.ExistingKaowoolfirebarriersinuseatSusquehannaSESarebeingmaintained.4.DateoFullComliance:BytakingtheactionsrequiredbyBulletin92-01andSupplement1,PPEcLisinfullcompliancewithThermo-Lagfirebarriermaterialrequirements.
