Report on PORV Failures

SUBJECT:

OPERATIONAL DATA ON PORV/SV FAILURES AND CHALLENGES In response to NUREG-0737 Item II.K.3.3, all PWR licensees committed to providing the NRC prompt notification of PORV/SV failures and periodic reports on PORV/SV challenges (annual or monthly reports). This reporting period began April 1, 1980.

Prior to the TMI accident, little data existed accurately portraying the operational failure and challenge rates of PORVs and in some cases SVs.

In order to better assess the validity of the licensee's responses to MPA F-37 " Report on PORV Failures" (generic owners group reports), it is requested that each PM for PWRs complete the attached questionaire and forward them to T. G. Colburn, MS 428 by COB April 26, 1983. PMs should have this information available in their records for the most part but if not, please contact your licensee to provide the requested information.

This request for information is approved by OMB under clearance number 3150-0065 which expires May 31, 1983..
