This inspection assessed the licensed operator requalification program. The inspection
consisted of reviews of training administrative procedures, medical records, requalification

training attendance records, and requalification training materials. The inspection included

j observation of certified evaluators and licensed operators during administration of the

i requalification examination. Interviews were conducted with members of the operations

and training staffs. )

. The licensed operator requalification program was being implemented in accordance with

l 10 CFR Part 55 requirements. The feedback process used to update the licensed operator ,

i requalification training program was not completely impiemented and was identified as a l

.] significant weakness (Section 05.4).

No significant errors during the inspection period were attributable to ineffective

training. The training program provided operators with skills necessary to safely

operate the Davis Besse Station.

number of questions to make the written examination discriminating. '

05.3 Reaualification Examination Administration Practices

The inspectors performed the following to assess the licensee's policies and

practices regarding requalification examination administration, simulator fidelity, and

examination security: ,

e Reviewed the licensee's administrative procedures

Evaluators assigned to conduct the examination were professional in their actions )

and demeanor at all times. Evaluators conducting JPMs provided initiating cues and

prompts correctly. Some suggestions for improving JPM administration were made l

by the Chief Examiner to the Operations Training Supervisor. Documentation of

crew dynamic simulator scenario participation and weaknesses was good. Although

'

documentation of individual operator weaknesses met minimum standards, it could

be improved to provide better feedback to the individual operators. Examination

security was demonstrated in all phases of examination administration.

The feedback system was considered a weakness in the licensee's training program.

Although instructors adequately documented individual and crew weaknesses

during the requalification period, a mechanism for evaluating the weaknesses and ,

providing that evaluation to the plant's training review committee was not

functioning. This prevented the training committee from accurately modifying the

training program to address the weaknesses that were discovered during operator

evaluations. Other portions of the feedback system, such as immediate feedback to

operator questions during classroom and simulator training sessions, were

functioning properly.

l It was determined the licensed operator requalification program was being

l implemented in accordance with 10 CFR Part 55 requirements. Adequate training

j was provided for the licensed operators to operate the Davis Besse Nuclear Power

l Station. Assessment and feedback on operator weaknesses to the Training Review

Council was determined to be a weakness.

The inspectors met with licensee representatives after the inspection on November

8,1996, to discuss the scope and findings of the inspection. During *he exit

meeting, the inspectors discussed the documents and processes rewmed by the

inspectors during the conduct of this inspection and the likely content of the final

inspection report. Licensee representatives did not identify any documents or

processes as proprietary.
