REQUESTED LIST OF RECENT PART 21 REPORTS In response to his telephone request, please transmit on the docket the attached computer listing of recent Part 21 reports to Rick Foote of the Houston Lighting & Power Company, the licensee for the South Texas Project.

To minimize the effort required to furnish this response, the 50-page listing is obtained from a standard report form in the NRC internal Events Tracking System.

The listing contains eight fields of use to the NRC staff involved with the review and evaluation of Part 21 related information. To assist Mr. Foote in learning of the concerns reported, we have ordered the individual entries by the vendor or site name. The fields with dates, assigned reviewer, Part 21 log number, and event type have administrative value only and probably will not be of use to non-NRC staff. The report number field may be of interest and use. This field may contain the accession number to identify the document ,

in the NRC Public Document Room (PDR), the number of an event notification report prepared by the NRC Operations Center based upon a phone call received by NRC, or the number of an internal report prepared by regional NRC staff based upon observations, etc. These internal reports are placed in the PDR but are not kept by the PDR.

The items are ordered by vendor name and contain a brief synopsis of each report. The first page of the attachment contains a legend explaining the l meaning of the several columns.

This part is a seven (7) position tap block located on the upper transformer printed circuit board. This part is used to change the relay's current settings. The tap block is designed to revert to the highest tap rating when the tap pin is removed. A small percentage (.0025) of these tap blocks were fourid to be defective from the ABB supplier. When the tap pin is removed the tap block will open circuit the transformer circuit. The transformer circuit is externally wired to the system current transformers. When the circuit transformers are open circuited a large voltage is possible, creating a hazard to the operator.

failure initiation echanism on Ellis and Watts safety-related coolers and a condenser. Specifically, the components identified as having damage are 1-PMcL-30-175-A(1A-A RHR ptsup room cooler), -COND-31-289 (480 V board room condenser 28-B), 1-PMCL-30-183-A (1A-A CCP room cooler A-A),

2-PMCL-30-190- A (CCS pump and Unit 1 AFW ptmp cooler A-A),

and 1-PMCL-30-191-B (CCS ptmp and Unit 1 AFW ptsup cooler B-8). Unit 1 was in cold shutdown, prior to Mode 6 (initial fuel loading).

The mechanism of failure was the bearing moving relative to the fan shaft. Overtensioning of belts was identified as the cause. Maintenance Instruction (MI)-0.16, " Maintenance Guidelines For Belt Driven Equipment," had been initiated in 1988 as a result of deficiencies written on air handling equipment. The instruction provided a belt tensioning method base on belt deflection (Browning Belt Manufacture guidance). This resulted in high forces / loads on the shaft and bearings.

Because of the identified condition, the required 100 days of continuous operation to support safe shutdown following a design basis accident may not be assured, since potential fan running time before failure could not be determined.

However, the fans had not failed prior to removal from service for repairs.

Forty one safety-related/ quality-related Ellis and Watts belt driven fans required for Unit 1 operation have been identified. As part of this review, eighteen were determined to be required to be inspected prior to mode 6.

Inspection has been performed on the eighteen units plus 2-Core-31-289. Five of the nineteen were identified to have damaged shafts and have been repaired. The corrective actions include a change to the method of tightening the belts for avoiding shaft damage.

To avert interference with crossover leg piping, the support eksigner at a nuclear power plant arranged for the base of the front inside support colum for a reactor coolant pump to be placed 6-12 inches closer to the reactor pressure vestci than in the original design. Westinghouse determined that the resulting 2-5 degree tilt in the column decreased the vertical stiffness by a smatt amount. More significantly, however, Westinghouse determined that it might degrade the thermal expansion and loop loadings of the system. Westinghouse identified the affected plants:
