INTRODUCTION The reactor operated continuously in August with the following exceptions: four shutdowns for scheduled maintenance and refueling; two unscheduled shutdowns.
MAINTENANCE ACTIVITIES, 8/4/02 Refueled - removed core 02-38, loaded core 02-39.
8/5/02 Refueled - removed core 02-39, loaded core 02-40.
Transferred eight spent fuel elements to the BMI cask in preparation for shipment to the Westinghouse Savannah River Site.
8/12/02 Refueled - removed core 02-40, loaded core 02-41.
8/19/02 Refueled - removed core 02-41, loaded core 02-42.
8/20/02 Refueled - removed core 02-42, loaded core 02-43.
8/26/02 Refueled - removed core 02-43, loaded core 02-44.
UNSCHEDULED SHUTDOWVNS Date No. Type Cause 8/4/02 1144 RRI Blade 'D' not in contact with magnet On August 4, a Rod Not In Contact With Magnet Rod Run-In was automatically initiated when Blade 'D' anvil separated from its magnet during a sample handling evolution. The Blade 'D' Full In Light was lit so the control room operator initiated a manual scram and the reactor was shutdown. It was determined that during sample handling an operator trainee had allowed a 3" silicon holder to bump Control Blade 'D' Offset Mechanism. This bump was sufficient to cause the anvil to separate from the magnet. The reactor was subsequently refueled and returned to power.
Date No. Typ. Cause 8/20/02 1145 RRI Blade 'A' not in contact with magnet On August 20, a Rod Not In Contact With Magnet Rod Run-In was automatically initiated when Blade 'A' anvil separated from its magnet during a sample handling evolution. The Blade 'A' Full In Light was lit so the control room operator initiated a manual scram and the reactor was shutdown. It was determined that during sample handling an operator trainee had allowed a 4" silicon holder to bump Control Blade 'A' Offset Mechanism. This bump was sufficient to cause the anvil to separate from the magnet. The reactor was subsequently refueled and returned to power.
Both of these unscheduled shutdowns were the result of relatively new operators handling samples in close proximity to the reactor control blades. Operations personnel were reminded of the necessity of flawless attention to detail and to use care in sample handling.
