TOI.EDO ED1 SON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-81-84 DATE OF EVENT: November 2, 1981 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Safety Features Actuation System (SFAS) Channel Func-tional Surveillance Requirement 4.3.2.1.1 was not performed on schedule Conditions Prior to Occurrence: The unit was in Mode 1 with Power (MWT) = 2634 and Load (Gross MWE) = 807.

Description of Occurrence: On November 2, 1981 at 0915 hours0.0106 days <br />0.254 hours <br />0.00151 weeks <br />3.481575e-4 months <br />, a surveillance schedule error was discovered by the Shift Supervisor. ST 5031.01, SFAS Monthly Test for SFAS Channel 4 was scheduled to be done out of its normal sequence. Following an investi-gation by the Surveillance Test Engineer, it was found that an incorrect completion date was entered for SFAS Channel 4. The actual completion date was September 21, 1981 while the entered completion date in the schedule was October 9, 1981. The Surveillance Requirement 4.3.2.1.1 for SFAS Channel 4 was exceeded. There were no unit power reductions because of this occurrence.

Designation of Apparent Cause of Occurrence: The cause of this occurrence was a per-sonnel error made when updating the computer test history. While entering the comple-tion date for SFAS Channel 2, a duplicate completion date was entered for SFAS Channel

4. A completion date of October 9, 1981, was entered for both Channels 2 and 4.

The actual completion date for Channel 4 was September 21, 1981. The computer projected a new test due date 28 days from the erroneous October 9 date instead of the September 21 date. This allowed an error of 18 days to be added to the SFAS Channel 4 due date.

Based on the September 21 date, the test due date should have been October 23, 1981, but due to the 18 day error. It was scheduled to be performed on November 12, 1981.

Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. The SFAS Monthly Test for Channel 4 was successfully per-formed on November 2, 1981, which confirmed the operability of Channel 4 since Sep-tember 21, 1981. In addition, the actual surveillance test late date for Channel 4 was October 30, 1981, based on the most conservative of the 3.25 and 1.25 percent of interval maximum allowable extensions. Therefore, the Surveillance Requirement 4.3.2.1.1 was exceeded by only 3 days.

Corrective Actlon: on November 2, 1981, at approximately 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> ST 5031.01, SFAS Monthly Test for SFAS Channel 4 was initiated and was successfully completed at  ;

approximately 10Yi honrn. SFAS Channel 4 was declared operable at 1145 hours0.0133 days <br />0.318 hours <br />0.00189 weeks <br />4.356725e-4 months <br /> on t November 2, 1981. The Surveillance Test Schedule was corrected to remove the in-correct completion date and restore Channel 4 to its normal sequence. Since this occuirence is considered to be an isolated event, no future corrective action is applicable.
