( .

, DATE OF EVENT: February 8, 1978 t

FACILITY: , Davis-Besse Unit'1 IDENTIFICATION OF OCCURRENCE: Replacement of Diesel Generator (DG) 1-1 turbo-charger due to anticipated failure. .

Conditions Prior to occurrence: The unit was in Mode 1, with Power (MWT) = 2060, and Load O!WF.) = 73C.

. Description of Occurrence: On February 8, 1978, while performing Surveillance Test ST 5081.01, " Diesel Generator Monthly Test", Operations personnel noticed that Diesel Generator (DG) 1-1 was making unusual noises which appeared to be originating from an air start motor. On further investigation by Maintenance personnel, it was determined that the noise was coming from the turbocharger.

The turbocharger puts more air into the diesel cylinders allowing better effi-ciency and higher horsepower. On a loss of the turbocharger, the diesel would not be able to reach full load pouar requirements. It was suspected the turbo-charger reliability was inadequate and the Shif t Foreman declared Dies:1 Genera-

/N tor 1-1 inoperable at 1245 hours0.0144 days
0.346 hours
0.00206 weeks
4.737225e-4 months
. This placed the unit in Action Statement "A"

( of Technical Specification 3.8.1.1, which requires-two ind'ependent Diesel Genern-Operations personnel test started Diesel Generator '

~

tors in Modes 1, 2, 3 and 4.

l-2 and performed Surveillance Test ST 5080.01, "offsite AC Sources" as required by the Action Staterent throughout the occurrence.

Designation of Ap3arent Cause of Occurrence: The apparent cause of this occur-rence is believed to be a component failure or a. design / fabrication error, but

~

l the exact nature of the problec is not known at this time. The turbocharger is being sent back to the factory where an investigation and analysis will be made.

Analysis of Occurrerce: There was no danger to the health and safety of the public ;

or unit personnel. Diesel Generator 1-2 was operable if a loss of normal power supplies had occurred.

Corrective Action: The vendor, Power Systems, was contacted and it was decided to replace the turbocharger. A service representative was sent to the site.

Diesel Generator 1-1 turbocharger was replaced under Maintenance Work Order 78-324 under the supervision of a vendor representative. Surveillance Test ST 5081.01, " Diesel Generator Monthly Test" was completed satisfactorily'and Diesel Generator 1-1 was declared operable at 1420 hours0.0164 days
0.394 hours
0.00235 weeks
5.4031e-4 months
on February 10, 1978, removing the unit from Action Statement A of Technical Specification 3.8.1.1.

'- Failure Data: There have been no previous reportable turbocharger failures.
