consumers Power GB Slade General Manager POWERINli MICHlliAN'S PROliRESS Palisades Nuclear Plant: 27780 Blue Star Memorial Highway, Covert, Ml 49043 October 7, 1993 Nuclear Regulatory Commission Document Control Desk Washington, DC 20555 DOCKET 50-255 - LICENSE DPR PALISADES PLANT - PRESSURIZER SAFE END CRACK ENGINEERING ANALYSIS AND ROOT CAUSE EVALUATION On September 16, 1993 a primary system through-wall crack developed in the pressurizer power operated relief valve (PORV) nozzle safe end. The crack was in the Inconel safe end connected to a stainless steel pipe. Since the time the crack was identified numerous inspections and evaluations have been completed to determine the root cause of the crack so that appropriate corrective actions can be taken to allow the Palisades Plant to be safely returned to service.

Our plans and progress for resolution of this issue has been the subject of many telephone conference calls and letters to the NRC dated September 29, 1993, October 1, 1993 and October 4, 1993. In the October 4, 1993 letter we stated that we would submit a report to the NRC describing the results of our actions. Enclosure 1 to this letter contains that report.

Enclosure 2 contains the formal Pressurizer Safe End Crack Repair Action Plan.

Also our October 4, 1993 letter contained an attachment which described the non-destructive examination we were completing in response to discovery of the crack. Part of that attachment was a two page table which summarized the NOE examinations planned for each of the welds beinef"examined and one sheet showing a cross section sketch of each of the three types of notzles that exist on the top of the pressurizer. Enclosure 3 contains updates to the table and nozzle sketches.
