  • TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEME';TAL INFORMATION FOR LER NP-33-80-73 DATE OF EVENT: August 12, 1980 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Non-Qualified Solenoid Valve Operator Installed on Con-1 tainment Purge Valve CV5005 Conditions Prior to Occurrence: The unit was in Mode 5 with Power (MRT) = 0 and Load '

(Gross MWE) = 0.

Description of Occurrence: On August 12, 1980 at 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br />, Instrument and Controls (I&C) personnel were comparing a suspected bad solenoid valve with a known good one, SV5005. This good valve, SV5005, controls the Containment Purge Inlet Isolation Damper CV5005. A check of I6C calibration and maintenance records showed that SV5005 was in-stalled August 26, 1976 for temporary use and was never replaced with the proper valve.

This temporary valve is not qualified for nuclear service. It is suited for its function since it is located outside of containment and would never be exposed to a LOCA environ-1 ment. Even during plant full power operation, the environment in the Purge Inlet Equip-ment Room No. 600 would not be considered severe. However, technically it was inopera-ble and was therefore secured in its safety position, closed.

Being a Safety Features Actuation System (SEAS)' Incident Level 1 valve, CV5005 must be operable in all modes, and therefore, the station entered the action statement of Tech-nical Specification 3.3.2.1. This action statement required the valve to be secured in its safety position, closed.

Designation of Apparent Cause of Occurrence: The cause was an inadequate implementation of an administrative control during the initial plant testing and system turnover.

I&C's Instrument Information Sheet shows that at the time-of the installation, it was 1l known that this solenoid valve was installed for. temporary use until a proper valve was received. The installation of this temporary valve without proper documentation was not in compliance with the administrative requirements of AD 1844.00, " Maintenance".

Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. The solenoid valve is located outside of containment and would never be exposed to a LOCA environment (which would mandate the nuclear service qualifi-cation). The solenoid valve is suitable for its function in the non-extreme environ-ment of Room No. 600. Its suitability is reinforced by the fact that it has functioned 1 properly since its installation. Also, solenoid valve SV5005 is normally de-energized and CV5005 is normally closed. The scienoid la energized to cause CV5005.to open.

Should the solenoid fail, it would always fail CV5005 to its safety position, closed.

In addition, the redundant penetracion 33 isolation valve CV5006 was also operable and was qualified for nuclear service. CV5005 could be further isolated by closing CV5004 (containment purge supply fan discharge) and CV5003B (inlet to mechanical penetration room #1). Therefore, containment integrity was not in question during this time.

-SUPPLEMENTAL INFORMATION FOR LER NP-33-80-73 PAGE 2-Corrective Action: An ongoing review of I&C records is being conducted to verify that there had been no other maintenance activities conducted during initial startup and testing that were completed without conforming to the administrative requirements of AD 1844.00. No additional discrepancies have been found.

-On August 12, l'980 when it was determined that the installed SV5005 solenoid valve was not qualified, CV5005 was declared inoperable and secured in its closed position.

The valve remained closed until a suitable replacement was installed. Nonconformance Report 292-80 was issued to ensure a qualified solenoid valve was used. Facility 1

Change Request 80-206 has been implemented which replaced SV5005 with an available nuclear service grade solenoid _ valve.

1 Failure Data: There have been no previous reports of the installation of a non-quali-fied solenoid valve.
