l-l- On September 13, 1989, at 0015 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br />, an hourly fire watch patrol exceeded the allowed interval. The patrol was started at 0017 hours1.967593e-4 days <br />0.00472 hours <br />2.810847e-5 weeks <br />6.4685e-6 months <br /> and was two minutes late. This is a viclation of Technical Specification 3.7.10, Action "a.2". i The employ.ee assigned to the watch failed to inform the Shift Supervisor immediately when his progress was impeded. His problem was quickly resolved, but not before. the allowed interval had passed.

The individual was counseled. All fire watch personnel received information on this incident and on their responsibilities as roving fire watches.

Descriotion of Occurrence:

On September.13, 1989, at 0015 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br />, an hourly fire watch patrol exceeded the-one hour interval between patrols of the Radiological Control Area (RCA). The

hourly patrol is required by Technical Specification-(TS)-3;7.10, Action "a.2" due to deficiencies in fire barriers in RCA. The hourly patrols are scheduled to begin on the hour. They are' performed at intervals of 60' minutes with a margin of 15 minutes. The patrol that was to start at 0000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> was not started until 0017 hours1.967593e-4 days <br />0.00472 hours <br />2.810847e-5 weeks <br />6.4685e-6 months <br />. This exceeded the 75 minutes maximum interval' by two minutes. This is a violation of Action Statement "a.2" of TS 3.7.10.

This is reportable under 10CFR50.73(a)(2)(1)(B) as a condition prohibited by

Technical Specifications.

Cause of Occurrence:

The cause was personnel error. The employee assigned to the fire watch did not conduct the watch as trained. The employee had some problems receiving.

-his self reading dosimeter at the entrance of RCA, but he failed'to contact-the Shift Supervisor immediately that his progress was impeded. By the time this was resolved, the patrol started two minutes beyond the allowed time interval.

I Anedvsis of Occurrence:

The hourly fire watch patrols are used as compensation when Technical Specification fire barriers are declared inoperable with operable fire detection on at least one side. The fire detection on at least one side of the barrier still provides early warning of a fire in the area or that fire has penetrated through'a barrier. The hourly fire watch patrol provides additional early'deteetion of fire problems. Being two minutes late for starting an' hourly fire watch patrol has no safety significance.

J The employee who started the watch late received counseling on September 21, 1989. All fire watch personnel received information on this event and a reminder of their responsibilities.

. Failure Data:

This is the first report of a late fire watch patrol s~ . .'R 88-014 (June 9, 1988) when a patrol was delayed by the testing of a r otation system.
