TOLEDO EDISON COMPANY t DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-79-02 DATE OF EVENT: January 2, 1979 FACILITY: Davis-Besse Unit 1 Channel 2 of the Reactor Protection System (RPS)

IDENTIFICATION OF OCCURRENCE:

_ Conditions Prior to Occurrence:

Load (Gross MWE) = 839.

Description of Occurrence: On January 2,1979, the Surveillance Test Coordinator dis-covered that ST 5030.02, "RPSlate Monthly Functional Test" had not yet been performed on date (maximum allowable extension) was January 2,1979 RPS Channel 2. The test at 0720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br />. Af ter verifying the testing interval was exceeded, the Surveillance Test Coordinator informed the Shif t Foreman, who then declared the channel inoperable at 1125 hours0.013 days <br />0.313 hours <br />0.00186 weeks <br />4.280625e-4 months <br />.

This placed the unit in the Action Statements of Technical Specification 3.3.1.1.

The channel was bypassed to perform the required testing within one hour of the time the channel was declared inoperable.

The Instrument and Control RPS Foreman was notified that the test would have to be per-Channels 1, 3, and 4 were operable at the time RPS Channel 2 formed immediately.

Corrective Action: At 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br />, on January 2, 1979, ST 5030.02 was started on RPS Channel 2. The Surveillance Test was completed and at 1925 hours0.0223 days <br />0.535 hours <br />0.00318 weeks <br />7.324625e-4 months <br />, the channel was declared operable.

To prevent recurrence, the Instrument and Control Foreman will now be issued a copy of Instrument and Control weekly schedule of required testing so the Instrument and Control Foreman can determine which tests are required without instructions from the Test Coordinator.

Failure Data:

There have been no previous occurrences of late surveillance testing caused by a breakdown in co=unications.
