LIC-1 5-0071 Attachment 1 Page 1 Revised Proposed Alternative Repairs to identified boric acid leakage sources will be made on RC-100, CEDM RC-10-24, and ICI Port #44 during the current refueling outage. Repairs to identified component cooling water (CCW) leakage sources will be made on CEDMs RC-10-03, RC-10-12, RC-10-14, RC-10-27, RC-1 0-35, and RC-1 0-39 during the current refueling outage. This corrects the known sources of leakage. Procedure OP-ST-RC-3007, Periodic Reactor Coolant System Integrity Test, which specifies inspection areas inside containment including the ICI Ports, CEDM housings, and RC-100, will be performed.

This surveillance test will verify the correction of the identified leaks and confirm the absence of new leaks.During Operating Cycle 28 OPPD will use administrative controls such that at an unidentified leak rate increase of greater than 0.1 gpm above stable baseline, actions will be taken to identify the source of the leakage. If the source is not identified within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, actions will be taken to shut the plant down.The "stable baseline" is the baseline mean defined in procedure, ER-AP-331-1003 "RCS Leakage Monitoring and Action Plan". ER-AP-331-1003 establishes the controls and expectations for monitoring Reactor Coolant System (RCS) leakage under the Exelon Nuclear Boric Acid Corrosion Control (BACC) program and the PWR Owner's Group (PWROG)Operations Subcommittee "Needed" Recommendations under letters OG-07-387

&OG-08-400, and associated guidance for these recommendations contained in WCAPs 16423 & 16465. It also defines the methodology used to establish acceptable baseline values.The "Stable Baseline" (Baseline Mean) is defined as a normal, steady state mean value for a given plant condition (gpm @ 60F to 70F and atmospheric pressure).

This is established during nominal Mode 1 plant conditions.

Following every outage, the previous quarter's (prior to the outage) Baseline Mean RCS unidentified leak rate value and Standard Deviation are used upon startup for a new fuel cycle or until such time as the plant systems become more stable and better values can be calculated.

Following a refueling outage it may take several weeks for the RCS leak rate to stabilize, because it takes time for equipment packing to swell, seals to run-in, etc. depending on the cycle. A revised baseline from the previous quarter is established based upon plant response post outage.The FCS experience over several outages (past 4), indicates that this takes nominally about one month. Following FCR27, it is expected that a "stable baseline" will be established approximately 4 weeks post startup.During Operating Cycle 28, OPPD will perform a bare-metal visual inspection on the upper RVH in accordance with the criteria contained in ASME Code Case N-729-1 on the first Cold Shutdown of greater than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> that occurs after at least 4 months of operation.

During Refueling Outage FCR28 OPPD will conduct UT/surface examinations of all RVH nozzles.

LIC-1 5-0071 Attachment 2 Page 1 Supplemental Response to NRC Request for Additional Information Regarding Relief Request RR-14 Question 3 NRC Question 3 For each nozzle which exhibited foreign material near the annulus, including staining, please identify the source of the material and provide a justification for the leakage being from that source (e.g., a chemical and/or isotopic analysis of the material at the annulus showing the material to be inconsistent with RCS chemistry; evidence that the material is non-adherent, or evidence that a leakage path exists from a point above each nozzle under consideration to a point below the nozzle such that it is reasonable to conclude that fluid would have entered the annulus of the nozzle under consideration).

Attachment 2 provides revised supplemental information to characterize and further clarify why we have reasonable assurance that the discovered deposits originated from other sources than the nozzles. This is shown in the revised attached Source Identification Diagram. This revised diagram depicts the dispersion of leakage from ICI-44, RC-10-24, RC-100, and the CCW flex hose. Also shown this diagram is the general flow of wash down liquid and where it tends to accumulate on the downhill side of various penetrations, particularly at the ICI locations.

Additionally, chemical analysis was obtained for both loose and adherent particles.

The results of this analysis indicates that the source of leakage is from sources other than nozzle.
