TOLEDO EDISON COMPANY DAVlS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFOR31ATION FOR LER NP-33-79-134 DATE OF EVENT: November 22, l'J79 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: An operator assigned to perform a fire watch failed to perform two hourly observations of his zone Conditions Prior to Occurrence: The unit was in Mode 1, with Power (MWT) = 2023, and Load (Gross MNE) = 685. Description of Occurrence: At 0001 hours on November 21, 1979, the remote contact data typer became inoperable, removing Control Room indication of fire alarms. To comply with the action statement of Technical Specification 3.3.3.8, an hourly fire watch was established to inspect all af fected zones. The hourly fire watch was main-tained until 0600 hours when the operator assigned the fire watch in the Emergency Diesel Generator Rooms was performing a surveillance test on the Diesel Fire Pump, and failed to perform the hourly fire watch. The fire watch was again performed on the Emergency Diesel Generator Rooms at 0800 hours. Designation of Apparent Cause of Occurrence: The root cause of this occurrence is a procedural deficiency. The log sheet used by the operator in AD 1810.00, Fire Pro-tection Program, did not adequately specify the surveillance requirements. The opera-tor believed the performance of the Diesel Fire Pump Surveillance Test was of higher priority than the fire watch. Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. Only two inspections of one fire zone were not performed. The local fire alarm panels were operable if a fire had occurred. Corrective Action: The fire watch was re-established at 0800 hours and maintained until the contact data typewriter was returned to operability at 1605 hours on Novem-ber 26, 1979. The applicable log sheet in AD 1810.00 will be modified to specify the technical specification requirements. Failure Data: There have been no previous failures to perform an established fire watch.
