Notification No. 15-1 Page l of`2 10 CFR Part 21 Report Defect Identified 6" Gear Operated Ball Valves (Part Number GBV-B2-60-0020 & GBV-B2-60-0112)

In accordance with I0 CFR Part 21.21(d)(4(i)-(ix), BNL Industries, Inc. (BNL) provides the following written notification:

(i) Name and address of the individual or individuals informing the Commission.

Vernon, CT 06066 (ii) Identification of the facility, the activity, or the basic component supplied for such facility or such activity within the United States which rails to comply or contains a defect.

The following are the basic components which are the subject of this notification:

First Energy - Perry -- (PO 45467911, Item 00001, Mat. No. 100100014)- P/N GBV-B2-60-0020 PC. 3, Serial Number A 150217- I -I (Trace Number 547G- I)

First Energy - Beaver Valley - (PO 45459398, Item 00023, Mat. No. 100095602) - GBV-B2-60-0112 PC.

3, Serial Numbers A 141009-23-3, A 141009-23-4, A 14,1009-23-5, A 141009-23-6 (Trace Number 547G- 1)

No addition defective material has been supplied and no other facilities are affected.

(iii) Identification of the firm constructing the facility or supplying the basic component which fails to comply or contains a defect.

Vernon, CT 06066 (iv) Nature of the defect or failure to comply and the safety hazard which is created or could be created by such defect or failure to comply.

On May 1,2015 BNL identified a discrepant condition in which material was identified by an audited third-party material testing facility as meeting the requirements of ASME SA-105, despite having a test yield strength of 35.6ksi (vice 36k required by the specification).

The discrepant material has already been manufactured and incorporated into a basic component and shipped to the end users.

Based on BNL engineering evaluations of the design criteria, code requirements and supplied material, BNL has determined there to be no safety hazard. Engineering evaluations EE-A 150217 Rev. 0 and EE-A 141009-23 Rev. 0 are being supplied to the purchasers.

(v) The date on which the information of such defect or failure to comply was obtained.

Page 2 of 2 (vi) In the case of a basic component which contains a defect or fails to comply, the number and location of these components in use at, supplied for, being supplied for, or may be supplied for, manufactured, or being manufactured for one or more facilities or activities subject to the regulations in this part.

First Energy - Beaver Valley - 7 endcaps supplied, GBV-B2-60-0112 PC. 3, Serial Numbers A 141009 3 (2pcs.), A141009-23-4 (2pcs.), A141009-23-5 (2pcs.), A141009-23-6 (lpc.) (Trace Number 547G-1)

(vii) The corrective action which has been, is being, or will be taken; the name of the individual or organization responsible for the action; and the length of time that has been or will be taken to complete the action.

Following identification of the discrepant material, BNL performed a review of all material test reports from the third part material testing facility. Trace number 547G-I was the only discrepant material found.

Concurrently, BNL performed engineering evaluations EE-A 150217 Rev. 0 and EE-A 141009-23 Rev. 0 to determine the impact on the design suitability. Based on the evaluations, the design is acceptable as is.

BNL has initiated a request for corrective action (RCA 0121), which will require all material test certifications to receive a secondary review. This action has been implemented immediately.

(viii) Any advice related to the defect or failure to comply about the facility, activity, or basic component that has been, is being, or will be given to purchasers or licensees.

The following Engineering Evaluations will be supplied to the purchaser:

First Energy - Perry - EE-A 150217 Rev. 0 First Energy - Beaver Valley - EE-A 141009-23 Rev. 0 (ix) In the case of an early site permit, the entities to whom an early site permit was transferred.

This is not an early site permit concern.
