As you know, the annual Fatigue Reporting Form has a

"Submission Update" feature, because the NRC staff understands that the reporting of Part

26 performance information may need updating due to on-going licensee corrective actions

and reviews.

With your submission update, please consider a very short description placed in a

"Summary of Corrective Actions" comment block on the Fatigue Form describing that

Southern Nuclear self-identified a reporting discrepancy (as you described below) in the

2013 submission and corrects the official record with this submission update to the original

2013 report.

You may use the NRC's Electronic Information Exchange submission portal, http://www.nrc.gov/site-help/e-submittals.html , for your updated submission as well.

This email and both reports (the original 2013 submission and submission update) will be

maintained as official records.

Mike Montecalvo, NRR, has the overall agency lead for the Subpart I fatigue management

provisions.

And Will Smith, NSIR, has the technical lead for assessing Security Officer

fatigue management implementation.

I reported that we had 7 fatigue assessments performed for post-events.

However, we discovered that one of the post-events was actually a For Cause fatigue assessment.

The correct info is "Six post-event and one For Cause fatigue assessments".

The Medical/FFD supervisor, performing the Fatigue Assessment, correctly classified the incident as a "For Cause" and also required a For Cause drug/alcohol test.

The Security Captain classified the incident as a "post-event" on the Fatigue Assessment

and in our Fatigue Management work hour tracking software, PQ&S, which is also used to manage the data on waivers, fatigue assessments, etc. I pulled the data from PQ&S for the 2013 report. I fully understand the importance of providing true and accurate information to the NRC. I plan to re-submit this report with the corrected information.

I am reporting this directly to you so that you are aware of the reason for the changes to the report.
