Previous Inspection Findings (Closed) Open Item (461/86045-07(DRS)):

This item addressed the environmental qualification (EQ) of jumper wires in limitorque actuators within the scope of 10 CFR 50.49.

The licensee had stated that all 10 CFR 50.49 designated EQ limitorque actuators contained Rockbestos Firewall III and Raychem Flametrol jumper wires, and that these jumper wires were qualified for accident conditions. The licensee could not identify the specific type of Firewall III wires installed in their actuators, but stated that they had qualification documentation on file to qualify the entire family of Rockbestos Firewall III wires.

The inspector reviewed EQ documentation for Firewall III, KXL-780, 760-D, 760-5, 760 and 760-G insulated cable.

With the exception of 760-G, all types of Firewall III have chemically cross linked Polyethylene insulation, 760-G is of the Irradiation Cross-Linked Polyethylene construction.

The cualification of KXL-760-0 has been demonstrated through Rockbestos Qualification Report No. QR-5804.

The qualification of 760-G has been demonstrated through QR-5805.

Both reports have been found acceptable by the NRC.

During this review the licensee presented a Rockbestos Test Report No. TR-6801 to demonstrate the similarity of KXL-780, KXL-760-5, KXL-760 to KXL-760-D.

This report addressed two tested samples of each of the above insulated cables for flammability, tensile strength and elongation.

Heat conditioned samples were subjected to an EM-60 accelerated water absorption test. The report concluded that test results indicated an overall similarity of the electrical and mechanical characteristics of the tested Firewall III samples.

The inspector observed that a LOCA Steam environment had not been used in this test to accurately simulate cable degradation.

The licensee reported that they had performed an extensive audit of Rockbestos on August 27 and 28, 1984, S&L Letter No. CQD-016039, and that this audit had helped mitigate concerns identified in IE 84-44 relative to cable purchased for the Clinton Plant.

In addition to this audit the licensee took credit for prior LOCA testing identified in the Rockbestos Report, " Qualification of Firewall III Class IE Electric Cables," Revision 0, dated July 7, 1977, and Revision 1, dated March 15, 1979.

Based on a review of qualification data in the Rockbestos Firewall III

,

reports, the Rockbestos similarity analysis TR-6801, and the licensee's Rockbestos audit addressing IE 84-44, the inspector had no further concerns regarding the qualification of the Firewall III wires used in Limitorque Actuators at the Clinton Plant.

Review of EQ Bulletins a.

(Closed) Bulletin (50-461/79001-BB)

This item addressed requirements of IE Bulletin No. 79-01, dated February 8, 1979, which required licensee's of operating plants to review the qualification of stem mounted NAMC0 limit switches in their plants, and to ensure the environmental qualification of safety-related equipment needed during an accident.

The licensee in a March 7, 1986, Inter Office Memo addressed concerns of this bulletin, and concluded that equipment important to safety in their plant was environmentally qualified.

During this inspection the inspector verified that the licensee did not have the deficient NAMC0 limit switches on their Master Equipment List (MEL), and that the licensee has a program to meet requirements of 10 CFR 50.49.

(Closed) Bulletin (50-461/79001-1B)

This item addressed requirements of IE Bulletin No.79-01A, dated June 6, 1979, and required licensees of operating plants to review the qualification of ASCO solenoid valves with BUNA "N" elastomers in their plant.

The licensee reviewed this bulletin for information only, and during this inspection the inspector verified that the licensee did not install the deficient ASCO Solenoid Valve Catalog No. HT-8331A45 in their plant.

This item addressed Supplement No. 1 of IE Bulletin No.79-01B, dated February 29, 1980, and included generic questions and answers resulting from a NRC Task Group / Licensee " workshop" meeting regarding the environmental qualifications of class 1E equipment in use at power reactor facilities.

The intent of this bulletin was to assist the licensees in their actions to provide evidence of equipment qualifications to the NRC.

No licensee response was required.

This item addressed Supplement 2 of IE Bulletin No.79-01B, dated September 30, 1980, and included generic questions and answers resulting from NRC/ Licensee meetings regarding environmental qualification of class IE equipment in use at power reactor facilities.

The intent of this bulletin was to clarify the scope and depth of the 79-018 review, and no specific response from licensees was required.

EQ Program Compliance to 10 CFR 50.49 As part of the 79-018 review the inspector reviewed the licensee's programmatic compliance to 10 CFR 50.49.

The NRC staff had performed an EQ review on March 11-14, 1985, at the Clinton Plant and in the FSAR concluded that Clinton Unit 1 had demonstrated full conformance with the regulatory requirements of 10 CFR 50.49.

During this review the inspector verified that the licensee had implemented their SER corrective action commitments; established procedures for implementing a program for the qualification of EQ equipment; established a controlled Master Equipment List; developed adequate methods for maintenance and replacement of EQ equipment; established auditable qualification files for Limitorque Actuators, File No. EQ-CL-009, Revision 19, and Rockbestos Cable, File No. EQ-CL-051, Revision 11; performed QA audits on EQ activities; and adequately addressed EQ Information Notices and Bulletins, including IE 83-72 concerns on Limitorque Actuators.

The inspector also examined EQ designated Limitorque Actuator No. IE 12-F03B at Elevation 803' inside

the containment. Wires examined were identified as Rockbestos Firewall III and Raychem Flametrol.

One jumper wire with a 'G' symbol on its insulation was verified by the licensee to be Rockbestos Firewall III from the Rockbestos East Granby Plant.

Heaters were found disconnected.

Adequate traceability was found for the qualification of the actuator motor and splices.

During this review the inspector did not find adequate evidence of a formal EQ training program for engineers, QA/QC inspectors and craftsmen performing EQ activities.

The licensee reported that this area was being reviewed for appropriate action.
