(A) Okay; so you said that your main feedwater pumps tripped?

(B) The main feedwater pumps tripped; the aux feedwater pumps either failed to initiate or tripped. At any rate, they weren't running when I came in.

(A) Okay, have you determined what the cause of that problem was?

(B) What's that?

l (A) Have you determined what the cause of the aux feedwater pump trip is? '

(B) No, we haven't. .

(A) Okay, is that still under investigation?

(B) That's still under investigation. They seem to be, seem to be controlling okay right now though, manually, so.....

(B) That we don't know. We had been having trcuble with them before and we have a whole lot of the instrumentation attached there. It vill be hours before we get to investigating that, so... We also don't have, we broke condenser vacuum because of drop in steam header pressure while we were trying to get back off the aux feed pump.

But in any case the aux feedwater pumps, they are operating right now, and they're operating normally.

(C) So they tripped and then they restarted them evidently.

(A) Well, this person appears did not know the information.

(C) Did they t.ip? Do we know that for sure?

(A) No, we don't.

(C) Okay. I'll just put down ' tripped?' for right now then. i

Evidently this person must have been the operator, STA, or something talk-ing to me; you know the other people are busy.

(C) Okay, so the cause was a feedwater problem, then. I (A) Right.

t (C) Other than that, that's about all 1 know about at this point. The reactor was tripped from 90% power, now it's in Hot Shutdown; Stable.

(B) Basically, I guess, we would have declared earlier on an Alert based on loss of--a :<.t Area Emergency based on the loss of all feedwater, but since it was uickly s restored before we could--within a few minutes before j

we could get into the emergency plan, we're not really its any obvious sec-tion of the emergency plan, but since we're uncertain as t.o what the cause of the loss of initiation or the lack of initiatior; of aux feedwater was, I

and since the main feedpumps are tripped still, the Shift Supervisor's a little uneasy about things and tt.at's why he wants some emergency plan activation. {

(C) Okay. Other than that the plant is still stable, and they're still inves-tigating the cause of the... either the pump tripped or failed to start. '

(A) Either it tripped or it failed to start.

(D) Hello.

(A) Yes, (name deleted). This is (name deleted), Headquarters.

(D) Yes.

I (A) Okay, Davis Besse dec,lared an Unusual Event due to a reactor trip and a bunch of weird things happened after the reactor tripped. The reactor trip was caused by a main feedwater pump tripping, which then isolated the main steam isolation valve. The problem arises when the aux feedwater -

pump failed to -either failed to start or---

(D) Whoa, whoa, whoa, hold on. The main feed pump tripped and that isolated the main steam isolation valve?

(A) Pardon me?

(D) The feedwater pump tripped and you said that automatically isolated the main steam isolation--

(A) Hey, 1 didn't say 'automatica11y'. That's what they told me. I sti'il -

that's one question I gotta ask them. Probably it...it's usually designed to isolate with less than 600 lbs of steam pressure.

(D) Right, it's a safety valve.

(A) And so, if you trip a pump.

06/13/85 16 DAVIS BESSE INCIDENT

C (D) If you trip a pump the pressure goes up. Alright, go ahead. What cise?

(A) Anyway, the problem arose when the aux feedwater pump either failed to start or it immediately tripped off after starting. But it had to be ini-tially manually initiated. Okay. And that occurred for about.5 minutes and they started their pumps up. And so the plant is stable,* everything is normal right now. The aux feedwater pump is still operating as normal but the cause of the problem on the aux feed pu ip is still undetermined at this time.

(D) And so the plant is stable.

(A) Uh, yes.

(D) Okay. And does the Region III Duty Officer know?

(A) Uh, ye.s he does.

(D) May, fine. Okay, thanks for calling, (name deleted).

(A) 0 ay, good enough.

Operations Officer for an update. (Region III) is very difficult to understand on the tape.

(E) Yes. Hi. (Name deleted) from RIII. I just got a call from my resident at Davis Besse...

(A) Yes.

(E) That they've had a trip.

i (A) Right.'  !

I (E) And, that he was going to go out and see what the devil was going on. He didn't get any detafis, but they'd gotten themselves in a tight fix. i (A) Yes.

I (B) What did the licensee tell you?

1 (A) Okay, now who's this speaking, please? I l

(A) Okay, (name deleted). They called me up on a reactor trip. I (E) Right.

(A) Let ma get my sheet out. Okay, the cause of the trip, which they told me on the second phone call, was due to main feedwater pump No. I tripping.

(E) Right, and they had the problem with the auxiliary feed pumps."

(A) Right. And they also had a main steam isolation valve isolation. Tne biggest problem was the aux feedwater pump had to be manually started be-cause it either failed, or failed to start, or tripped. And that cause is still under investigation.

(E) Did they declare it an Unusual Event?

(A) Yes, they did. This was after I questioned the fact that the person who was '.alking to me knew very little information, and it took a third phone ca'.1 in order to declare an Unusual Event. I asked him what the event classification was, and he said at that time he didn't know. .

(E) I was just looking in our NRC book and it says that one of the initiating conditions for notification of an Unusual Event would be loss of engi- ,

neered safety feature or fire protection system function requiring shut-l down by technical specification. I would say that any danger of losing '

auxiliary feedwater would automatically qualify as an Unusual Event.

(A) Right. But in their emergency plan the loss of the aux feedwater system is declared a site aret emergency.

(E) Oh? But did they say they declarec a cite area emergency?

(A) No, they didn't. Okay, within a couple minutes they started the pumps, and by that time they weren't even in their emergency plan.

(E) 1s the plant stable?

(A) V:s. The plant is stable and the aux feedwater system was operating prop-erly within, I think, five minutes, they mentioned, after the reactor trip, (E) Well, anyway I thought I'd call to let you know that (name da1eted) and (name deleted) both probably be there within a half hour to forty five minutes.

(A) That's probably a good idea because the person I talked to knew very lit-t1e information, and they seemed pretty disoriented when I talked to him on the phone.

(E) Right, I think, did you tell the Region III Duty Officer.....

(A) Yes, I did.

(E) When d,id you call him? At Home?

i (A) Pardon me?

(E) When did you call him? We need to follow up. I'd like to suggest you call him back when I hang up.

(A) Okay. I called him about 15 minutes ago.

(A) What was the cause of the isolation of your main steam isolation valves?

(8) I do not know.

(A) It's still under investigation?

(B) There were...we had no conditions at the time that would have caused those to close.

(A) Okay. Have you deters.ined the cause of your main er aux feedwater from tripping?

(A) Yes, good morning, your status.

(B) Okay. Right now we have the stortup feed pump running and it's supplying feedwater to both steam generators. We have both aux pumps, feedwater pumps available but not pumping water. Right now we're in the process of reestablishing vacuum in the condenser and normal post-trip lineup.

(A) Ohay,,Any new news on all the trips?

(B) All what trips?

i I (A) Oh, the cause of the main feedwater...

t (8) 0h, okay. No, there's no... we've got some chart recorders that have data on them. We're analyzing it. The feed pump did trip and appears to be on

a... well, we've got indication that it may have been on overspeed.

(A) O kay.

1 (B) We have no... we do understand wt had a w' ,+ call a steam and feedwater ,

j rupture control system trip concurrent with the feed pump trip. And that was what isolated... did the isolation and secured the other main feed pump, t .

(A) Oh, okay.  !

(B) And then both aux feedwater pumps started but tripped due to problems with their trip throttle valve.

(A) Okay. So you're telling me the trip throttled this valve? *

(B) We originally thought it was overspeed because that's the only thing that really trips them but we're, we're thinking, we're having problems with the trip throttle valve. And once they were ...the trip throttle valves were recet they've been running. We've had one system that has not, one auxiliary feedwater system that we've declared inoperable, although it's available; we just don't have the proper control.

(A) Okay. You still in your Unusual Event?

(B) 8e're still on the unusual event. We could get out of it, but we'd like to stay in it until we get vacuum restored and get off of our safety ,

systems.

(A) Okay. It sounds pretty good.

(B) Okay.

(A) Thanks for the info.

. 1 (A) Right. And they suspect that to be the problem of why it tripped off. But they're still investigating.

(C) Okay. i (A) Okay, the M51V closure, OK, is still a mystery. But, apparerdly it was actuated by the feed steam leak isolation system.

j (A) Okay, the cause of the main steam isolation closure was a result of the  ;

(A) But it's still a mystery why it actually actuated. '

therefore they lost all main feedwater. Remember that the whole event

] (A) Right. They thought it was overspeed but that's the only thing that can trip these things, but they're looking at it a little closely, and they think that it could be a problem with the trip throttle valve. But when they reset it, everything was fine.
