TheSacramento Municipal UtilityDistrictrecentlyreportedaproblemencountered withtheoperation oftheGEModelCR105Xauxiliary contactmechanism installed ina480voltcircuitbreaker.Investigation intothecausefortheinability toshutdownaboostersupplyfan(SF-A-7)inthecontrolroomemergency airconditioning systemrevealedthatanauxiliary contact(GEModelCR105X)hadfailedintheclosedposition, preventing thefan'spowersupplycircuitbreakerfromopening.Thespecificcauseforfailurewasbindingoftheplungerarmduetoburrsandnicksonitssurface.Aninvestigation wasconducted bythelicenseetodetermine theextentofusageofthistypeauxiliary contactinothercircuitsthroughout thereactorpowerplant.Approximately fifty(50)positions inthenuclearservicemotorcontrolcenterswereidentified ashavingasimilartypeauxiliary contactmechanism.
Itwasalsodetermined thatmanyoftheaffectedsystemswhichrequirecontactoperation similartothatdescribed above,eitherpermitorprovideasafetyfeaturefunctionduringemer-gencyconditions.

TheSacramento Municipal UtilityDistrictrecentlyreportedaproblemencountered withtheoperation oftheGEModelCR105Xauxiliary contactmechanism installed ina480voltcircuitbreaker.Investigation intothecausefortheinability toshutdownaboostersupplyfan(SF-A-7)inthecontrolroomemergency airconditioning systemrevealedthatanauxiliary contact(GEModelCR105X)hadfailedintheclosedposition, preventing thefan'spowersupplycircuitbreakerfromopening.Thespecificcauseforfailurewasbindingoftheplungerarmduetoburrsandnicksonitssurface.Aninvestigation wasconducted bythelicenseetodetermine theextentofusageofthistypeauxiliary contactinothercircuitsthroughout thereactorpowerplant.Approximately fifty(50)positions inthenuclearservicemotorcontrolcenterswereidentified ashavingasimilartypeauxiliary contactmechanism.

Itwasalsodetermined thatmanyoftheaffectedsystemswhichrequirecontactoperation similartothatdescribed above,eitherpermitorprovideasafetyfeaturefunctionduringemer-gencyconditions.

Anexampleofthistypeapplication isauxiliary

,contactsthatmustopentopermitclosingofcertainsafetyrelatedvalvesfrom480voltmotorcontrolcenters.TheattachedGEServiceAdviceLetterandassociated instruction/drawing sheetweresenttoallnuclearpowerreactorfacilities byGEFieldServiceOffices.Theletter,togetherwiththeinstruction/drawing sheetidentifies theproblemandprovidestherecommended corrective action.
