I reported that we had 7 fatigue assessments performed for post-events. However, we discovered that one of the post-events was actually a For Cause fatigue assessment. The correct info is "Six post-event and one For Cause fatigue assessments". | |||
The Medical/FFD supervisor, performing the Fatigue Assessment, correctly classified the incident as a "For Cause" and also required a For Cause drug/alcohol test. | |||
The Security Captain classified the incident as a "post-event" on the Fatigue Assessment and in our Fatigue Management work hour tracking software, PQ&S, which is also used to manage the data on waivers, fatigue assessments, etc. I pulled the data from PQ&S for the 2013 report. I fully understand the importance of providing true and accurate information to the NRC. I plan to re-submit this report with the corrected information. I am reporting this directly to you so that you are aware of the reason for the changes to the report. | |||
