PRELIMINARY NOTIFICATION - REGION III October 27, 2014 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE - PNO-III-14-009A This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. Some of the information may not yet be fully verified or evaluated and is basically all that is known by the Region III staff on this date.

UPDATE TO UNPLANNED SHUTDOWN GREATER THAN 72 HOURS DUE TO A LOSS OF FEEDWATER This Preliminary Notification provides an update to PNO-III-14-009 for a shutdown of greater than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> of Perry following an automatic scram from 100 percent power on October 20, 2014. The scram occurred, as designed, following the unexpected shutdown of the main feedwater pumps. The licensee was shifting the electrical power supply of a nonsafety-related component when the pumps shut down.

The licensee investigated the cause of the shutdown of the main feedwater pumps and did not conclusively identify a human error or failed component. The licensee did, however, re-align

equipment to bypass the nonsafety-related component involved with the loss of the pumps.

The licensee restarted the reactor on October 25 and connected the generator to the offsite electrical distribution grid on October 26. The resident inspectors observed portions of these activities.

The State of Ohio has been notified.
