PRELIMINARY NOTIFICATION - REGION III November 12, 2014 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE - PNO-III-14-011 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. Some of the information may not yet be fully verified or evaluated and is basically all that is known by the Region III staff on this date.

UNPLANNED SHUTDOWN GREATER THAN 72 HOURS DUE TO A LOSS OF FEEDWATER On November 7, 2014, at 8:47 a.m. EST, the Perry reactor automatically shut down from 100 percent power, as designed, following an unplanned automatic reduction in feedwater flow.
Feedwater flow to the reactor is needed to make the steam that ultimately generates electricity.
All safety-related equipment responded as designed to the shutdown.
The licensee replaced several components in the digital control system for feedwater flow that were identified by the licensee and the control system vendor as needing replacement. On November 11 at 4:07 a.m. EST, the Perry reactor was restarted following these replacements and subsequent testing. The plant was reconnected to the offsite electrical distribution grid at 6:15 a.m. EST on November 12.
Region III received initial notification of this occurrence from the Senior Resident Inspector at Perry on November 7 shortly after the reactor shutdown. The Resident Inspector proceeded to the control room to review the shutdown and both resident inspectors closely monitored the plant cooldown, repair activities, and subsequent startup of the reactor. A similar reactor shutdown involving the feedwater system occurred on October 20, 2014. The resident inspectors and regional specialists are reviewing both shutdowns.
The State of Ohio has been notified.
