Some of the information may not yet be fully verified or evaluated and is basically all that is known by the Region III staff on this date.

SUBJECT:

UNPLANNED SHUTDOWN GREATER THAN 72 HOURS DUE TO SPURIOUS OPENING OF SAFETY RELIEF VALVES AND SUBSEQUENT PLANT RESTART On February 8, 2016, at 1500 EST, with the Perry Nuclear Power Plant at 100 percent power, two safety relief valves (SRVs) opened upon a spurious initiation signal. This caused the suppression pool temperature to increase. At 1503 EST, operators at the plant manually scrammed the reactor when the suppression pool temperature reached 95 degrees Fahrenheit in order to ensure margin to the heat capacity limit required for shutdown reactor safety by Technical Specifications. During the scram, all control rods fully inserted and all equipment functioned normally. The SRVs closed immediately following the scram.

Following the scram, the licensee cooled down and depressurized the reactor to perform troubleshooting activities. The licensee identified the spurious opening was caused by gas intrusion into the reactor pressure reference leg system. Therefore, the licensee completed venting of the reference leg system and instrument lines connected to the reference leg to ensure that there is no air in the system.

Following the repair activities, licensee personnel restarted the reactor on February 12, 2016, and reached full power on February 17, 2016. There was no impact on public health and safety.

The State of Ohio has been notified.

This preliminary notification is issued for information only.

The resident inspectors were on site and responded to the control room to monitor the plant parameters and licensee actions for the scram, and observed the subsequent troubleshooting and repair activities.

The information presented herein has been discussed with the licensee, and is current as of 1036 EST on February 18, 2016.
