UNPLANNED SHUTDOWN GREATER THAN 72 HOURS DUE TO A PRESSURE BOUNDARY LEAK.
On June 16, 2013 at 0200 EDT, the Perry Nuclear Power Plant commenced a controlled plant shutdown due to a pressure boundary leak. Perry reduced power to around 8 percent on Friday, June 14, 2013, to make a drywell entry and investigate an increasing drywell leakage trend. The leak was identified on June 15, 2013 at 2250 EDT, when steam was observed coming from a three-quarter inch vent line off the top of the 'B' Reactor Recirculation Flow Control Valve. The licensee evaluated the leak rate as approximately 0.3 gpm and it appeared to be coming from a weld flaw on the inlet to the inboard vent valve. The leak was subsequently determined to meet Technical Specification 3.4.5, Action (C) which requires the plant to be in Mode 3 within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.
The leakage was contained in the plant. There was no impact on public health.
The State of Ohio has been notified.
This preliminary notification is issued for information only.
Region III received initial notification of this occurrence when the licensee called the Senior Resident Inspector at 2330 EDT on June 15, 2013. The NRC resident staff monitored Perrys actions to identify the leak and the subsequent repairs.
The information presented herein has been discussed with the licensee, and is current as of 0800 EDT on June 18, 2013.
